【摘要】：目的:對比分析子宮動脈栓塞(UAE)聯(lián)合化療和單純化療在保守性治療剖宮產(chǎn)瘢痕妊娠(CSP)的效果及相關(guān)影響因素。方法:選取2003年1月~2014年12月在中山大學附屬第一醫(yī)院確診CSP且拒絕手術(shù)治療的60例患者,其中22例行單純MTX化療,38例行UAE聯(lián)合化療。結(jié)果:單純MTX化療組中6例(27.3%)失敗,化療失敗者的治療前血β-HCG平高于化療成功者(P=0.001)。UAE聯(lián)合化療組中2例(5.3%)治療失敗,住院時間、β-HCG轉(zhuǎn)陰、正常月經(jīng)恢復時間均較單純MTX化療組顯著縮短(P0.001),術(shù)后血β-HCG轉(zhuǎn)陰及正常月經(jīng)恢復與治療前血β-HCG、孕周、妊娠囊大小等因素無相關(guān)性。結(jié)論:UAE聯(lián)合化療較單純MTX化療更有效,術(shù)后康復較快,且不受患者個體因素的影響。而治療前高β-HCG是導致單純MTX化療失敗的相關(guān)因素。
[Abstract]:Objective: to compare and analyze the effect and related factors of uterine artery embolism (UAE) combined with chemotherapy and chemotherapy alone in conservative treatment of (CSP) in cesarean section scar pregnancy. Methods: from January 2003 to December 2014, 60 patients with CSP diagnosed in the first affiliated Hospital of Sun Yat-sen University were selected, including 22 patients with MTX chemotherapy and 38 patients with UAE combined chemotherapy.
