【摘要】：目的探討不同的斷臍時(shí)機(jī)對(duì)母兒結(jié)局的影響。方法選取河北醫(yī)科大學(xué)第一醫(yī)院2013年1-12月分娩的400例產(chǎn)婦隨機(jī)分為四組:早斷臍組,晚斷臍1組、晚斷臍2組、晚斷臍3組,每組各100例,比較斷臍時(shí)機(jī)對(duì)產(chǎn)婦產(chǎn)后的出血量、產(chǎn)婦血紅蛋白值、新生兒Apgar評(píng)分、新生兒生后72 h血紅蛋白值、新生兒膽紅素峰值的影響。結(jié)果早斷臍組產(chǎn)婦產(chǎn)后的出血量、產(chǎn)后血紅蛋白值、新生兒1 min和5 min Apgar評(píng)分及新生兒膽紅素峰值與晚斷臍組比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05);三種晚斷臍組之間產(chǎn)婦產(chǎn)后的出血量、產(chǎn)后血紅蛋白值、新生兒1 min和5 min Apgar評(píng)分及新生兒膽紅素峰值比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05);晚斷臍組新生兒生后72 h毛細(xì)血管血紅蛋白值高于早斷臍組(P0.05);三種晚斷臍組之間新生兒生后72 h毛細(xì)血管血紅蛋白值比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論三種晚斷臍方法均增加了新生兒生后72 h血紅蛋白值,不增加新生兒膽紅素水平,對(duì)新生兒是有益的,未顯示晚斷臍對(duì)產(chǎn)婦有不良影響,分娩應(yīng)遵循自然規(guī)律,等待胎盤娩出后斷臍,以防止過早斷臍可能潛在的不良影響。
[Abstract]:Objective to investigate the effect of different time of umbilical cord breakage on maternal and fetal outcome. Methods 400 parturients who gave birth in the first Hospital of Hebei Medical University from January to December 2013 were randomly divided into four groups: early umbilical cord group, late umbilical cord weanding group 1, late umbilical cord transection group 2 and late umbilical cord transection group, each group (n = 100). The effects of umbilical cord breaking time on postpartum bleeding volume, maternal hemoglobin value, neonatal Apgar score, neonatal hemoglobin value and neonatal bilirubin peak value were compared. Results there was no significant difference in postpartum bleeding volume, postpartum hemoglobin value, neonatal 1 min and 5 min Apgar score and neonatal bilirubin peak value between the early umbilical cord group and the late umbilical cord group (P 0.05), but there was no significant difference in postpartum bleeding volume, postpartum hemoglobin value, neonatal 1 min and 5 min Apgar score and neonatal bilirubin peak value between the three late umbilical cord groups (P 0.05), but there was no significant difference between the three late umbilical cord groups (P 0.05), but there was no significant difference in postpartum bleeding volume, postpartum hemoglobin value, neonatal 1 min and 5 min Apgar score and neonatal bilirubin peak value between the three late umbilical cord groups (P 0.05). The value of capillary hemoglobin in the late umbilical cord group was higher than that in the early umbilical cord group at 72 hours after birth (P 0.05), but there was no significant difference among the three late umbilical cord groups at 72 hours after birth (P 0.05), but there was no significant difference in the capillary hemoglobin value among the three late umbilical cord groups at 72 h after birth (P 0.05). Conclusion the three methods of late umbilical cord transection can increase the hemoglobin value at 72 hours after birth, and do not increase the neonatal bilirubin level, which is beneficial to the newborn. It is not shown that the late umbilical cord transection has a negative effect on the parturient. Delivery should follow the natural law and wait for the placenta to break the umbilical cord after delivery in order to prevent the potential adverse effects of premature umbilical cord breakage.
