胎盤凋亡及細(xì)胞轉(zhuǎn)運(yùn)與新生兒PBMC HBsAg關(guān)系

【摘要】:目的探討胎盤凋亡及外周血單個(gè)核細(xì)胞(PBMC)母-胎轉(zhuǎn)運(yùn)與HBs Ag陽性孕婦新生兒PBMC HBs Ag的關(guān)系。方法用等位基因特異性PCR(As-PCR)篩選母兒信息病例對,用原位末端雜交法檢測HBs Ag陽性孕婦足月胎盤凋亡指數(shù),用免疫熒光雙標(biāo)法檢測新生兒PBMC涂片上的HBs Ag和谷胱甘肽S轉(zhuǎn)移酶(GST);采用SPSS 16.0軟件進(jìn)行分析。結(jié)果 86例新生兒PBMC中20例(23.26%)HBs Ag陽性,31例(36.05%)GST陽性,HBs Ag、GST共存者13例(15.12%);發(fā)生PBMC母-胎轉(zhuǎn)運(yùn)組的胎盤凋亡指數(shù)與未發(fā)生轉(zhuǎn)運(yùn)組的胎盤凋亡指數(shù)差異有統(tǒng)計(jì)學(xué)意義(t'=2.38,P=0.02),且胎盤滋養(yǎng)層細(xì)胞凋亡率高與PBMC轉(zhuǎn)運(yùn)相關(guān)(t=2.75,P=0.01);新生兒PBMC HBs Ag陽性組的胎盤細(xì)胞凋亡指數(shù)與陰性組的胎盤細(xì)胞凋亡指數(shù)差異無統(tǒng)計(jì)學(xué)意義(t=0.34,P=0.74);PBMC轉(zhuǎn)運(yùn)與新生兒PBMC HBs Ag陽性有關(guān)(χ2=9.48,P=0.00),發(fā)生PBMC轉(zhuǎn)運(yùn)的新生兒發(fā)生PBMC HBs Ag陽性的危險(xiǎn)性是未發(fā)生PBMC轉(zhuǎn)運(yùn)新生兒的4.95倍(OR=4.95,95%CI=1.70~14.39)。結(jié)論妊晚期胎盤細(xì)胞凋亡增加有利于PBMC母-胎轉(zhuǎn)運(yùn),感染HBV的PBMC可能通過胎盤進(jìn)入胎兒血循環(huán)進(jìn)而導(dǎo)致新生兒HBV感染。
[Abstract]:Objective to investigate the relationship between placental apoptosis and maternal-fetal transport of (PBMC) in peripheral blood mononuclear cells (PBMCs) and neonatal PBMC HBs Ag in HBs Ag positive pregnant women. Methods the maternal and infant information cases were screened by allele specific PCR (As-PCR). The apoptosis index of term placenta of HBs Ag positive pregnant women was detected by in situ end hybridization, and the HBs Ag and glutathione S transferase (GST); on neonatal PBMC smears were detected by immunofluorescence double standard method.
