【摘要】：目的:比較3種不同劑量來(lái)曲唑(letrozol,LE)促排卵效果及對(duì)內(nèi)膜、性激素的影響。方法:選擇Ⅱ型排卵障礙的不孕患者92例,隨機(jī)分為3組,第1組(30例)及第2組(32例)在月經(jīng)第3天分別給予來(lái)曲唑2.5 mg/d、5 mg/d,共5 d,第3組(30例)在月經(jīng)第3天頓服來(lái)曲唑20 mg。觀察人絨毛膜促性腺激素(human chorionic gonadotropin,HCG)注射日的卵泡成熟天數(shù)、成熟卵泡數(shù)目、子宮內(nèi)膜厚度、血雌二醇(estradiol,E2)、黃體生成素(luteinizing hormone,LH)、總睪酮(testosterone,T)水平、排卵率、未破卵泡黃素化綜合征(luteinized unruptured follicle syndrome,LUFS)的發(fā)生率及排卵后7 d血E2、孕酮水平。結(jié)果:3組間HCG注射日的卵泡成熟天數(shù)、成熟卵泡數(shù)目、子宮內(nèi)膜厚度、血E2、LH、T水平、排卵率以及排卵后7 d血孕酮水平差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。3組間LUFS發(fā)生率、排卵后7 d血E2水平差異有統(tǒng)計(jì)學(xué)意義。結(jié)論:3組方案均能有效促進(jìn)卵泡發(fā)育,HCG日內(nèi)膜厚度均達(dá)到0.8 cm以上,排卵后血孕酮水平均高于60 nmol/L。僅LE 20 mg單次口服組排卵后7 d血E2水平可達(dá)自然生理周期黃體期正常水平,而總體給藥劑量較低,使用方便,推薦可作為L(zhǎng)E促排卵首選方案。
[Abstract]:Objective: to compare the effects of three different doses of letrozole (letrozol,LE) on ovulatory induction and intima and sex hormones. Methods: 92 infertile patients with type II ovulatory disorder were randomly divided into three groups. Group 1 (n = 30) and group 2 (n = 32) were given letrozole 2.5 mg/d,5 mg/d, for 5 days on the 3rd day of menstruation, and group 3 (n = 30) received letrozole 20 mg. on the 3rd day of menstruation. The days of follicular maturation, the number of mature follicles, the thickness of endometrium, the level of serum estridiol (estradiol,E2), luteinizing hormone (luteinizing hormone,LH), total testosterone (testosterone,T), ovulatory rate, the incidence of (luteinized unruptured follicle syndrome,LUFS) and the levels of E2 and progesterone 7 days after ovulatory were observed. Results: there were no significant differences in follicular maturation days, number of mature follicles, intimal thickness, blood E2, LH, T level, ovulatory rate and serum progesterone level 7 days after ovulatory among the three groups (P 0.05). There was significant difference in the incidence of LUFS among the three groups and the level of serum E2 7 days after ovulatory. Conclusion: all the three groups can effectively promote follicular development. The intimal thickness of HCG is above 0.8 cm, and the level of progesterone in blood after ovulatory is higher than 60 nmol/L.. Only LE 20 mg single oral group could reach the normal level of normal corpus phase 7 days after ovulatory, but the overall dose was low and easy to use. It is recommended that it can be used as the first choice for LE ovulatory induction.
