【摘要】：目的:編制《圍生期孕婦健康素養(yǎng)量表》,為圍生期孕婦健康素養(yǎng)測量工具的進(jìn)一步開發(fā)提供依據(jù);探索重慶市某區(qū)圍生期孕婦健康素養(yǎng)及其影響因素,為進(jìn)一步開展圍生期孕婦健康素養(yǎng)干預(yù),提高圍生期孕婦健康素養(yǎng)水平提供依據(jù)。方法:根據(jù)Nutbeam D教授提出的功能性、溝通性、批判性3大健康素養(yǎng)層次為理論支撐,通過大量查閱國內(nèi)外文獻(xiàn),并經(jīng)課題組多次討論,以及對18位專家開展咨詢調(diào)查,形成包含58個條目的圍生期孕婦健康素養(yǎng)初始量表;然后于2016年3-5月在1所綜合醫(yī)院和2所婦幼保健院門診選取477名圍生期孕婦,采取7種條目篩選方法對初始量表進(jìn)行條目篩選,形成包含51個條目的圍生期孕婦健康素養(yǎng)正式測試量表,并對其可行性、信度和效度進(jìn)行初步考評。采取整群抽樣方法于2016年9-11月抽取1所綜合醫(yī)院和2所婦幼保健院門診523名圍生期孕婦,運(yùn)用《圍生期孕婦健康素養(yǎng)量表》進(jìn)行現(xiàn)況調(diào)查,用Epidata3.1建立數(shù)據(jù)庫,并用SPSS17.0進(jìn)行圍生期孕婦健康素養(yǎng)水平及其影響因素的分析。結(jié)果:專家咨詢結(jié)果顯示,專家積極系數(shù)為90%,平均權(quán)威系數(shù)為0.87。通過條目篩選和信效度檢驗,形成包含功能性健康素養(yǎng)、溝通性健康素養(yǎng)和批判性健康素養(yǎng)3個領(lǐng)域11個維度共51個條目組成的《圍生期孕婦健康素養(yǎng)量表》。經(jīng)考評,該量表回收率為95.4%,完成率為100%,完成時間都在20分鐘內(nèi);總量表及三個領(lǐng)域的Cronbach'sα系數(shù)分別為0.909、0.879、0.848、0.843,分半信度系數(shù)分別為0.885、0.802、0.773、0.823;三個領(lǐng)域與總量表的相關(guān)系數(shù)在0.690-0.784之間,因子分析顯示量表結(jié)構(gòu)與理論構(gòu)想基本相符�，F(xiàn)況研究顯示,功能性、溝通性、批判性、總健康素養(yǎng)具備率分別為35.8%、16.1%、7.1%、14.0%;鄉(xiāng)鎮(zhèn)、學(xué)歷初中及以下、丈夫?qū)W歷初中及以下、家庭月收入4000元及以下、不按時孕期體檢、不愿意接受產(chǎn)前教育培訓(xùn)、性格內(nèi)向、與公婆及配偶關(guān)系不好的圍生期孕婦健康素養(yǎng)具備率較低(P均0.05)。多因素logistic回歸分析表明,圍生期孕婦的學(xué)歷、是否按時孕期體檢、是否愿意接受產(chǎn)前教育培訓(xùn)是功能性健康素養(yǎng)的影響因素;是否愿意接受產(chǎn)前教育培訓(xùn)、與公婆的關(guān)系是溝通性健康素養(yǎng)和總健康素養(yǎng)的影響因素;家庭月收入、與公婆的關(guān)系是批判性健康素養(yǎng)的影響因素。結(jié)論:《圍生期孕婦健康素養(yǎng)量表》具有較好的可行性、信度、效度,可作為測評圍生期孕婦健康素養(yǎng)的工具。圍生期孕婦健康素養(yǎng)水平受多種因素影響,針對這些影響因素采取相應(yīng)的干預(yù)措施是下一步工作的重點(diǎn)。
[Abstract]:Objective: to compile the Perinatal Maternal Health Nutrition scale (PHS), and to provide evidence for the further development of health literacy measurement tools for parturient pregnant women. To explore the health literacy of pregnant women in a district of Chongqing and its influencing factors, so as to provide the basis for further intervention of health literacy of pregnant women in the perioperative period and the improvement of the level of health literacy of pregnant women in the postpartum period. Methods: according to the theoretical support of functional, communicative and critical levels of health literacy put forward by Professor Nutbeam D, through a large number of references at home and abroad, and after many discussions by the research group, and 18 experts were consulted and investigated. The initial scale of health literacy of pregnant women in the postpartum period was formed, which included 58 items. Then, from March to May 2016, 477 pregnant women were selected from 1 general hospital and 2 maternal and child health centers, and seven kinds of item screening methods were used to screen the items of the initial scale. A formal test scale for maternal health literacy was formed, which included 51 items, and its feasibility, reliability and validity were evaluated. From September to November 2016, 523 postpartum pregnant women were selected from 1 general hospital and 2 maternal and child health centers by cluster sampling. The current situation was investigated by using the Perinatal Maternal Health element scale, and the database was established by Epidata3.1. SPSS17.0 was used to analyze the health literacy level and its influencing factors of postpartum pregnant women. Results: the results of expert consultation showed that the positive coefficient of experts was 90% and the average authority coefficient was 0.87. Through item screening and reliability and validity test, 51 items including functional health literacy, communicative health literacy and critical health literacy were formed, which included 51 items in three fields: functional health literacy, communicative health literacy and critical health literacy. The recovery rate of the scale was 95.4%, the completion rate was 100%, and the completion time was within 20 minutes. The Cronbach's 偽 coefficients of the total scale and the three fields were 0.909, 0.879, 0.848 and 0.843, respectively, and the split-half reliability coefficients were 0.885, 0.802, 0.773 and 0.823, respectively.
