【摘要】：目的:探討小劑量阿司匹林對(duì)于重度宮腔粘連電切術(shù)后子宮內(nèi)膜容受性的影響。方法:采用前瞻性隨機(jī)單盲的方法,將我院宮腔鏡下確診為重度宮腔粘連并實(shí)施宮腔粘連電切術(shù)+宮腔球囊置入術(shù)后的125例患者隨機(jī)分為研究組(A組)和對(duì)照組(B組),A組65例,術(shù)后給予補(bǔ)佳樂3 mg,3次/d+阿司匹林75 mg,1次/d,連續(xù)治療1個(gè)月;B組60例,給予補(bǔ)佳樂3 mg,3次/d,連續(xù)治療1個(gè)月。觀察2組患者排卵日子宮內(nèi)膜厚度、子宮內(nèi)膜類型、子宮內(nèi)膜容積、子宮動(dòng)脈血流阻力指數(shù)(resistance index,RI),子宮動(dòng)脈搏動(dòng)指數(shù)(pulsed index,PI),術(shù)后l、3個(gè)月復(fù)查宮腔鏡檢查,隨訪妊娠結(jié)局。結(jié)果:(1)A組患者排卵日子宮內(nèi)膜厚度較B組厚(P=0.000),A型內(nèi)膜所占比例高于B組(P=0.004),差異具有顯著性;RI,PI明顯低于B組,差異具有顯著性(P=0.000)。(2)2組患者子宮內(nèi)膜容積差異無(wú)顯著性(P=0.474)。(3)術(shù)后3個(gè)月A組宮腔形態(tài)恢復(fù)總有效率為60.0%(39/65),B組宮腔形態(tài)恢復(fù)總有效率為41.7%(25/60)(P=0.040),差異有統(tǒng)計(jì)學(xué)意義。(4)術(shù)后隨訪6~12個(gè)月,A組中35例試孕,已妊娠15例,B組21例試孕,已妊娠6例(P=0.033)。結(jié)論:重度宮腔粘連電切術(shù)后小劑量阿司匹林聯(lián)合大劑量補(bǔ)佳樂口服可以明顯改善其子宮內(nèi)膜容受性,提高術(shù)后試孕患者妊娠率。
[Abstract]:Objective: to investigate the effect of low dose aspirin on intimal receptivity after resection of severe uterine adhesions. Methods: 125 patients with severe uterine adhesion diagnosed by hysteroscopy in our hospital were randomly divided into study group (group A) and control group (group B). Group A (n = 65) was treated with Bujiale 3 mg,3 / d aspirin 75 mg,1 / d for 1 month. Group B (n = 60) was treated with Bujiale 3 mg,3 / d for 1 month. The intimal thickness, intimal type, intimal volume, uterine artery blood flow resistance index (resistance index,RI) and uterine artery pulsatility index (pulsed index,PI) on ovulatory day were observed.
