【摘要】：目的:探討低分子肝素聯(lián)合硫酸鎂治療妊娠期高血壓的臨床效果。方法:選取中山市博愛醫(yī)院2014年5月至2016年6月收治的88例妊娠期高血壓患者,隨機(jī)分為治療組與對(duì)照組。對(duì)照組采取單一硫酸鎂治療,治療組則采取低分子肝素聯(lián)合硫酸鎂治療。觀察并對(duì)比兩組患者的療效情況、記錄兩組患者治療前后的血小板計(jì)數(shù)以及凝血酶原時(shí)間、測(cè)定紅細(xì)胞壓積與D-二聚體的結(jié)果、統(tǒng)計(jì)產(chǎn)后2 h內(nèi)出血量情況。結(jié)果:治療后,兩組患者的紅細(xì)胞壓積與D-二聚體水平均較治療前有明顯改善,治療組患者的紅細(xì)胞壓積與D-二聚體水平明顯低于對(duì)照組患者,差異具有統(tǒng)計(jì)學(xué)意義(P0.05);兩組患者治療前與治療后的血小板計(jì)數(shù)以及凝血酶原時(shí)間比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);治療組患者產(chǎn)后2 h內(nèi)的出血量與對(duì)照組比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:針對(duì)妊娠期高血壓患者,對(duì)其采取低分子肝素聯(lián)合硫酸鎂治療的效果明顯優(yōu)于單一實(shí)施硫酸鎂治療,能夠顯著緩解患者體內(nèi)的高度血凝狀態(tài)的同時(shí)不增加患者的凝血障礙與產(chǎn)后出血量。
[Abstract]:Objective: to investigate the clinical effect of low molecular weight heparin (LMWH) combined with magnesium sulfate on hypertensive complicating pregnancy. Methods: from May 2014 to June 2016 in Boai Hospital of Zhongshan City, 88 patients with hypertensive complicating pregnancy were randomly divided into two groups: treatment group and control group. The control group was treated with single magnesium sulfate, while the treatment group was treated with low molecular weight heparin combined with magnesium sulfate. The curative effects of the two groups were observed and compared. The platelet count and prothrombin time were recorded before and after treatment. The hematocrit and D-dimer were measured. The bleeding volume within 2 hours after delivery was counted. Results: after treatment, the levels of hematocrit and D-dimer in the two groups were obviously improved, and the levels of hematocrit and D-dimer in the treatment group were significantly lower than those in the control group. The difference was statistically significant (P0.05); There was no significant difference in platelet count and prothrombin time between the two groups before and after treatment (P0.05). There was no significant difference in the bleeding volume between the treatment group and the control group within 2 hours after delivery (P0.05). Conclusion: low molecular weight heparin (LMWH) combined with magnesium sulfate is superior to magnesium sulfate alone in the treatment of hypertensive complicating pregnancy. It can significantly relieve the high hemagglutination state of the patient without increasing the coagulation disorder and postpartum hemorrhage.
