【摘要】：目的:探討南京地區(qū)年輕女性人乳頭瘤病毒(HPV)流行病學(xué)現(xiàn)狀,旨在為HPV感染的防治提供科學(xué)依據(jù)。方法:回顧2014年1月至2015年12月在中大醫(yī)院婦產(chǎn)科門診及相關(guān)科室就診并行HPV檢測的896例年輕女性(18~30歲)的臨床及病理資料,并初步探討年輕人群中HPV感染與宮頸病變的相關(guān)性。結(jié)果:在納入的896例年輕女性中,HPV陽性者共281例,占31.36%。高危型人乳頭瘤病毒(hrHPV)檢出率最高的前5種型別分別為HPV52、HPV16、HPV53、HPV39及HPV58;低危型人乳頭瘤病毒(lr HPV)檢出率最高的前3種型別分別為HPV81、HPV6及HPV11。132例年輕患者行陰道鏡下活檢,根據(jù)組織病理74例納入宮頸良性病變組,平均年齡為26歲;19例納入低級別宮頸鱗狀上皮內(nèi)病變(LSIL)組,平均年齡為27.3歲;34例納入高級別宮頸鱗狀上皮內(nèi)病變(HSIL)組,平均年齡28.2歲;5例確診為宮頸癌,實足年齡均為30歲。不同級別的宮頸病變組間存在年齡差異,且有統(tǒng)計學(xué)意義(P0.05)。在伴有HPV感染的年輕女性中,HSIL的發(fā)生率為45.68%,顯著高于不伴HPV感染的年輕女性(3.92%),差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:南京地區(qū)年輕女性最常見的hrHPV分別為HPV52、HPV16、HPV53、HPV39及HPV58。伴有hrHPV感染的年輕女性,隨著年齡增長,更易出現(xiàn)HSIL。
[Abstract]:Objective: to investigate the epidemiological status of human papillomavirus (HPV) in young women in Nanjing, in order to provide scientific basis for the prevention and treatment of HPV infection. Methods: the clinical and pathological data of 896 young women (18 years old) who were examined by HPV from January 2014 to December 2015 in gynaecology and obstetrics department and related departments of CUHK Hospital were retrospectively reviewed. And to explore the correlation between HPV infection and cervical lesions in young population. Results: of the 896 young women included, 281 (31.36%) were HPV positive. The first five types of high risk human papillomavirus (hrHPV) were HPV52,HPV16,HPV53,HPV39 and HPV58;, respectively. The first three types with the highest detection rate of low-risk human papillomavirus (lr HPV) were HPV81,HPV6 and HPV11.132, respectively, who underwent colposcopy biopsy. According to histopathology, 74 cases were included in the benign cervical lesions group, the average age was 26 years. 19 cases were enrolled in low-grade cervical squamous intraepithelial lesion (LSIL) group, the average age was 27.3 years, 34 cases were enrolled in high-grade cervical squamous intraepithelial lesion (HSIL) group, the average age was 28.2 years. Five cases were diagnosed as cervical cancer with full age of 30 years old. There was age difference between different grades of cervical lesions, and there was statistical significance (P0.05). Among the young women with HPV infection, the incidence of HSIL was 45.68%, which was significantly higher than that of young women without HPV infection (3.92%), the difference was statistically significant (P0.05). Conclusion: the most common hrHPV of young women in Nanjing is HPV52,HPV16,HPV53,HPV39 and HPV58., respectively. Young women with hrHPV infection are more likely to develop HSIL. as they age
