【摘要】：目的探討應(yīng)用不同方法處理腹腔鏡下卵巢囊腫剝除術(shù)后創(chuàng)面對殘留卵巢功能的影響。方法選取新疆生產(chǎn)建設(shè)兵團(tuán)醫(yī)院婦科收治的120例卵巢囊腫患者,行腹腔鏡下剝除術(shù)。依對創(chuàng)面給予的相應(yīng)止血治療方法隨機(jī)分為3組:電凝組、超聲組與縫合組,每組各40例。檢測3組患者術(shù)前、術(shù)后即時(shí)、術(shù)后3個月、術(shù)后6個月時(shí)月經(jīng)第2天外周靜脈血中促卵泡素(FSH)、黃體生成素(LH)、雌二醇(E2)水平,并行經(jīng)陰道超聲測定雙側(cè)卵巢儲備卵泡數(shù)量。同時(shí)記錄患者術(shù)后相應(yīng)時(shí)間月經(jīng)變化及臨床表現(xiàn)。結(jié)果術(shù)后電凝組患者FSH、LH、E2水平變化最為明顯,超聲組患者次之,縫合組患者變化最小且恢復(fù)最快(P0.05)。電凝組患者術(shù)后儲備卵泡數(shù)量減少最明顯,且不良反應(yīng)發(fā)生率較高。結(jié)論對腹腔鏡下卵巢囊腫剝除術(shù)后的創(chuàng)面,應(yīng)用電凝止血對殘留卵巢功能影響最大,超聲止血與縫合止血對其功能保護(hù)較好。
[Abstract]:Objective to investigate the effect of different methods on residual ovarian function after laparoscopic ovarian cyst exfoliation. Methods 120 cases of ovarian cyst were treated by laparoscopy in the gynecological department of Xinjiang production and Construction Corps Hospital. The wounds were randomly divided into three groups: electrocoagulation group, ultrasound group and suture group, with 40 cases in each group. The levels of follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2) in peripheral venous blood were measured before, immediately after operation, 3 months after operation and 6 months after menstruation in three groups, and the levels of follicle stimulating hormone and luteinizing hormone (LH), in peripheral venous blood were measured on the 2nd day after menstruation. The number of bilateral ovarian reserve follicles was measured by transvaginal ultrasound. At the same time, the changes of menstruation and clinical manifestations were recorded at the corresponding time after operation. Results the change of FSH,LH,E2 level was the most obvious in the electrocoagulation group, the second in the ultrasonic group, and the smallest and the fastest in the suture group (P0.05). In the electrocoagulation group, the number of reserve follicles decreased most obviously, and the incidence of adverse reactions was higher. Conclusion for the wounds after laparoscopic ovarian cyst exfoliation, electrocoagulation has the greatest effect on residual ovarian function. Ultrasonic hemostasis and suture hemostasis have better protection on the residual ovarian function.
