【摘要】：目的：探討單孔腹腔鏡下子宮切除術(shù)(LESS-TH)的應(yīng)用價(jià)值方法：研究篩選廣西壯族自治區(qū)人民醫(yī)院2013年1月至2015年1月期間,因婦科非惡性疾病需行子宮切除術(shù)的120例患者,隨機(jī)分配60例為實(shí)驗(yàn)組,實(shí)施單孔腹腔鏡下子宮切除術(shù)；60例為對(duì)照組,實(shí)施傳統(tǒng)腹腔鏡下子宮切除術(shù)。統(tǒng)計(jì)兩組的手術(shù)時(shí)間、術(shù)中出血量、術(shù)后肛門排氣時(shí)間、術(shù)后病率、住院時(shí)間、術(shù)后疼痛評(píng)分等相關(guān)數(shù)據(jù),并進(jìn)行分析比較。結(jié)果：兩組手術(shù)均順利完成。兩組均無(wú)術(shù)中、術(shù)后嚴(yán)重并發(fā)癥,無(wú)手術(shù)死亡病例,無(wú)1例中轉(zhuǎn)開(kāi)腹手術(shù)。實(shí)驗(yàn)組和對(duì)照組的手術(shù)時(shí)間分別為(98.91±18.97)min和(90.60±15.73)min；術(shù)中出血量分別為(65.00±21.75)ml和(67.36±38.02)ml;住院時(shí)間分別為(7.57±0.98)天和(7.13±1.01))天；術(shù)后病率分別為5%和8.3%,四組資料統(tǒng)計(jì)差異均無(wú)明顯統(tǒng)計(jì)學(xué)意義(P0.05)。實(shí)驗(yàn)組和對(duì)照組的術(shù)后疼痛評(píng)分分別為(1.41±0.27)分和(1.61±0.27)分,兩組資料統(tǒng)計(jì)差異有明顯差異(P0.05)。實(shí)驗(yàn)組和對(duì)照組的術(shù)后肛門排氣時(shí)間分別為(44.00±9.34)h和(49.07±5.00)h,兩組資料統(tǒng)計(jì)有明顯差異(P0.05)。術(shù)后3個(gè)月隨訪,兩組患者均無(wú)遠(yuǎn)期并發(fā)癥。實(shí)驗(yàn)組和對(duì)照組患者對(duì)腹部傷口美容滿意度分別為(4.57±0.51)分和(4.07±0.54)分,其差異有明顯的統(tǒng)計(jì)學(xué)差異(P0.05),實(shí)驗(yàn)組患者滿意度明顯優(yōu)于對(duì)照組。結(jié)論：?jiǎn)慰赘骨荤R手術(shù)是一種安全、可行的手術(shù)方式,其因術(shù)后疼痛減少,術(shù)后恢復(fù)更快,有更佳的美容效果,更易被患者所接受。
[Abstract]:Objective: to evaluate the value of single hole laparoscopic hysterectomy (LESS-TH) in the study of 120 patients undergoing hysterectomy for gynecological non-malignant diseases from January 2013 to January 2015 in the people's Hospital of Guangxi Zhuang Autonomous region. 60 cases of experimental group were randomly assigned to perform single-hole laparoscopic hysterectomy. 60 cases as control group were treated with traditional laparoscopic hysterectomy. The operation time, blood loss, postoperative anal exhaust time, postoperative disease rate, hospitalization time and postoperative pain score were analyzed and compared between the two groups. Results: the operation was completed successfully in both groups. In both groups, there were no severe postoperative complications, no operative death and no conversion to open surgery. The operative time of experimental group and control group were (98.91 鹵18.97) min and (90.60 鹵15.73) min;, respectively. The amount of intraoperative bleeding was (65.00 鹵21.75) ml and (67.36 鹵38.02) ml;, respectively. The hospitalization time was (7.57 鹵0.98) days and (7.13 鹵1.01) days, respectively. The postoperative disease rates were 5% and 8.3% respectively. There was no statistical difference among the four groups (P0.05). The postoperative pain scores of the experimental group and the control group were (1.41 鹵0.27) and (1.61 鹵0.27), respectively. There was significant difference between the two groups (P0.05). The postoperative anal exhaust time in the experimental group and the control group was (44.00 鹵9.34) h and (49.07 鹵5.00) h, respectively. There was significant difference between the two groups (P0.05). There were no long-term complications in both groups after 3 months follow-up. The beauty satisfaction of experimental group and control group were (4.57 鹵0.51) and (4.07 鹵0.54), respectively, the difference was statistically significant (P0.05), the patient satisfaction of experimental group was significantly better than that of control group. Conclusion: single hole laparoscopic surgery is a safe and feasible method, which can be easily accepted by patients because of the reduction of postoperative pain, the faster recovery after operation and the better cosmetic effect.
