【摘要】：目的為高齡非輸卵管性不孕女性推薦更合理的助孕方案。方法選擇行宮腔內(nèi)人工授精(intrauterine insemination,IUI)和體外受精-胚胎移植(in vitro fertilization and embryo transfer,IVF-ET)的1 312名高齡輸卵管通暢不孕女性(年齡≥35歲患者)共2 094個(gè)周期進(jìn)行回顧性分析,其中IUI組827個(gè)周期,IVF組啟動(dòng)周期1 267個(gè),移植周期798個(gè)。根據(jù)患者的年齡層次分為35~37歲,38~39歲和≥40歲3組,回顧性分析各組的臨床資料和助孕結(jié)局。結(jié)果 3個(gè)不同年齡層的IUI組臨床妊娠率分別為10.78%、8.72%和4.14%,IVF-ET組的移植周期臨床妊娠率分別為53.03%、47.51%和22.69%,IUI和IVF組內(nèi)及組間各年齡層臨床妊娠率比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論隨著患者年齡的增加,IUI和IVF兩種助孕方案的臨床妊娠率均明顯降低,建議年齡≥40歲輸卵管通暢的不孕女性應(yīng)積極行IVF治療。
