【摘要】：目的探討子宮動脈臨時阻斷(temporary uterine artery blocking,TUAB)在腹腔鏡子宮手術(shù)中的應(yīng)用效果。方法回顧性分析2015年1月~2016年10月實(shí)施TUAB的65例資料,其中剖宮產(chǎn)瘢痕妊娠36例,輸卵管間質(zhì)部妊娠16例,子宮肌瘤13例(巨大子宮肌瘤5例,闊韌帶肌瘤3例,子宮側(cè)壁肌瘤3例,宮頸肌瘤2例)。先用10號絲線打活結(jié)阻斷雙側(cè)子宮動脈,常規(guī)完成手術(shù),抽去結(jié)扎線。結(jié)果 65例手術(shù)均順利完成,行宮、腹腔鏡聯(lián)合剖宮產(chǎn)瘢痕妊娠物清除+子宮修補(bǔ)術(shù)36例,腹腔鏡輸卵管間質(zhì)部妊娠物清除+子宮修補(bǔ)術(shù)16例,腹腔鏡子宮肌瘤剔除術(shù)13例,術(shù)中出血量分別為(35.0±24.1)ml、(40.0±27.6)ml、(72.3±43.0)ml,手術(shù)時間(83.1±23.3)min、(86.8±19.9)min、(91.5±24.3)min,術(shù)后住院時間(5.2±1.0)d、(4.9±0.6)d、(5.2±1.0)d。術(shù)中無大出血發(fā)生。術(shù)后盆腔感染1例(1.5%),無其他并發(fā)癥發(fā)生。結(jié)論在有大出血風(fēng)險的腹腔鏡子宮手術(shù)中行TUAB手術(shù)安全,值得推廣。
[Abstract]:Objective to investigate the effect of temporary occlusion of uterine artery (temporary uterine artery blocking,TUAB) in laparoscopic uterine surgery. Methods from January 2015 to October 2016, 65 cases of TUAB were retrospectively analyzed, including 36 cases of cesarean scar pregnancy, 16 cases of tubal interstitial pregnancy, 13 cases of uterine leiomyoma (5 cases of giant hysteromyoma, 3 cases of broad ligament leiomyoma). There were 3 cases of uterine lateral myoma and 2 cases of cervical myoma. The bilateral uterine arteries were blocked by 10-wire ligation, then the ligation line was removed. Results 65 cases were successfully completed, 36 cases underwent uterine repair with laparoscopy combined with cesarean scar removal, 16 cases with laparoscopic tubal interstitial pregnancy removal, 13 cases with laparoscopic hysteromyomectomy. Intraoperative bleeding volume was (35.0 鹵24.1) ml, (40.0 鹵27.6) ml, (72.3 鹵43.0) ml, operation time (83.1 鹵23.3) min, (86.8 鹵19.9) min, (91.5 鹵24.3) min, Postoperative hospitalization time was (5.2 鹵1.0) d, (, 4.9 鹵0.6) d, (, 5.2 鹵1.0 d). No major bleeding occurred during the operation. Pelvic infection occurred in 1 case (1.5%), and no other complications occurred. Conclusion TUAB is safe and worth popularizing in laparoscopic uterine surgery with the risk of massive bleeding.
