【摘要】：目的：本研究旨在觀察補腎法聯(lián)合HCG治療腎虛型黃體功能不全性不孕癥的臨床療效，，探討其中西醫(yī)結(jié)合治療的作用機制。 方法：將60例中醫(yī)辨證為腎虛型、西醫(yī)診斷為黃體功能不全所致的不孕癥患者隨機分為治療組和對照組各30例，治療組采用補腎養(yǎng)精湯聯(lián)合HCG治療，對照組采用六味地黃軟膠囊、還少膠囊及HCG治療，3個月為一個療程，觀察患者的臨床療效及用藥前后的臨床癥狀、基礎(chǔ)體溫、內(nèi)分泌激素水平以及B超監(jiān)測卵泡、子宮內(nèi)膜等變化。 結(jié)果：補腎法聯(lián)合HCG治療腎虛型黃體功能不全所致的不孕癥的臨床妊娠率為77%，在整體證候改善方面，總有效率為93%，均明顯高于對照組（P0.05）�；颊呋A(chǔ)體溫以及高溫相評分均有明顯的改善（P0.05）。治療組在黃體中期血清孕酮水平明顯高于對照組，兩組間差異有顯著性（P0.05）。在改善子宮內(nèi)膜厚度方面，治療后子宮內(nèi)膜厚度明顯增厚（P0.05）。在影響卵泡發(fā)育方面，治療后卵泡直徑明顯增大（P0.05）。 結(jié)論：補腎中藥治療腎虛型黃體功能不全性不孕癥，可以通過調(diào)節(jié)自身的下丘腦-垂體-卵巢軸的功能，來促進卵泡成熟發(fā)育、卵巢黃體的生成，改善內(nèi)分泌條件，增加子宮內(nèi)膜的厚度，明顯縮短基礎(chǔ)體溫的移行時間，增加高溫相持續(xù)時間及其穩(wěn)定性等。補腎法聯(lián)合HCG治療黃體功能不全性不孕癥有很好的臨床療效，明顯提高臨床妊娠率，對內(nèi)分泌有很好的調(diào)節(jié)作用。
[Abstract]:Objective: to observe the clinical effect of tonifying kidney method combined with HCG in the treatment of luteal insufficiency infertility of kidney deficiency type, and to explore the mechanism of combination of traditional Chinese and western medicine. Methods: sixty cases of infertility caused by luteal insufficiency were randomly divided into treatment group (n = 30) and control group (n = 30). The treatment group was treated with Bushen Yangjing decoction combined with HCG. The control group was treated with Liuwei Dihuang soft capsule, Xiaoxiao capsule and HCG for a course of three months. The clinical effect and clinical symptoms, basic body temperature, endocrine hormone level and B-ultrasound monitoring of follicles were observed. Changes in the endometrium. Results: the clinical pregnancy rate of kidney tonifying method combined with HCG in treating infertility caused by luteal insufficiency of kidney deficiency type was 77. The total effective rate of improving the overall syndromes was 93, which was significantly higher than that of the control group (P0.05). Patients with basic body temperature and hyperthermia score were significantly improved (P0.05). The serum progesterone level in the middle luteal phase in the treatment group was significantly higher than that in the control group, and the difference between the two groups was significant (P0.05). In terms of improving endometrial thickness, endometrial thickness was significantly thickened after treatment (P0.05). The follicle diameter increased significantly after treatment (P0.05). Conclusion: Chinese medicine for tonifying the kidney can promote follicular maturation, luteal formation and endocrine condition by regulating the function of hypothalamus-pituitary-ovarian axis in the treatment of renal deficiency type luteal insufficiency infertility. Increasing the thickness of endometrium, shortening the migration time of basic body temperature, increasing the duration of hyperthermia and its stability, etc. Kidney tonifying method combined with HCG in the treatment of luteal insufficiency infertility has a good clinical efficacy, significantly improve the clinical pregnancy rate, and has a good endocrine regulation.
