【摘要】：在S.Matsubara等人對(duì)我們?cè)凇墩憬髮W(xué)學(xué)報(bào)(英文版)B輯:生物醫(yī)學(xué)與生物技術(shù)》發(fā)表的一篇報(bào)道"疤痕子宮產(chǎn)前出血可能是子宮動(dòng)脈瘤的潛在危險(xiǎn)信號(hào)"(Zhang et al.,2017)的評(píng)論(Matsubara et al.,2017)中,對(duì)于子宮動(dòng)脈瘤形成的機(jī)制給予了兩種解釋,進(jìn)而結(jié)合其所在中心的研究數(shù)據(jù)和文獻(xiàn)報(bào)道,提出子宮動(dòng)脈瘤的形成可能與所有既往的創(chuàng)傷性操作史有關(guān),兩者間的時(shí)間間隔可能很長(zhǎng),比如10年甚至20年之久。這個(gè)結(jié)論對(duì)子宮動(dòng)脈瘤的預(yù)防和診療等具有非常重要的指導(dǎo)價(jià)值。筆者查閱相關(guān)領(lǐng)域的文獻(xiàn),發(fā)現(xiàn)絕大多數(shù)研究者把創(chuàng)傷性操作的定義默認(rèn)為當(dāng)前妊娠分娩中所發(fā)生的操作,這樣的定義容易讓讀者忽視既往創(chuàng)傷性操作史與子宮動(dòng)脈瘤形成間的相關(guān)性。因此,筆者建議,在患者病例信息中,應(yīng)該明確既往創(chuàng)傷性操作史的背景介紹,便于提高臨床對(duì)子宮動(dòng)脈瘤發(fā)生的警惕性,并有利于對(duì)子宮動(dòng)脈瘤發(fā)生機(jī)制的研究。另外,臨床工作者需要對(duì)疤痕子宮的產(chǎn)前出血特別重視,其可能是子宮動(dòng)脈瘤發(fā)生破裂的潛在危險(xiǎn)信號(hào)。
[Abstract]:S.Matsubara et al. Report to us in the Journal of Zhejiang University, Series B: biomedicine and Biotechnology, "Hemorrhage of the scar uterus before birth may be a potential danger signal for uterine aneurysms" (Zhang et al., In 2017 (Matsubara et al.,2017), two explanations for the formation of uterine aneurysms were given, which combined with research data and literature reports at the center of the aneurysm. It is suggested that the formation of uterine aneurysm may be related to all previous traumatic operations, and the time interval between the two may be very long, such as 10 or 20 years. This conclusion has very important guiding value for the prevention and treatment of uterine aneurysm. The author looked up the literature in related fields and found that most researchers define traumatic operations by default as those occurring during pregnancy and childbirth. Such a definition would allow readers to overlook the correlation between previous traumatic procedures and uterine aneurysm formation. Therefore, the author suggests that the background of previous traumatic operation should be clearly defined in the patient's case information, so as to improve clinical vigilance against uterine aneurysm and to study the pathogenesis of uterine aneurysm. In addition, clinical workers need to pay special attention to prenatal bleeding of scar uterus, which may be a potential risk signal for rupture of uterine aneurysms.
