【摘要】：正患者,34歲,因孕31+6周,下腹脹痛伴呼吸困難13+小時(shí)于2015年6月13日19時(shí)急診入院。患者平素月經(jīng)規(guī)律,末次月經(jīng):2014年11月2日。孕期未定期產(chǎn)前檢查。無頭痛、心悸、氣促、皮膚瘙癢及雙下肢水腫等癥狀。自訴平素喜食肥肉,長期進(jìn)食油膩食物,4天前受涼后有上呼吸道感染癥狀,表現(xiàn)為流涕、咽痛,無畏寒、發(fā)熱、咳嗽等,自行口服藥物(具體不詳)治療后癥狀緩解。1天前出現(xiàn)尿頻、尿急、尿痛、尿不盡感,當(dāng)?shù)蒯t(yī)院予鹽酸利托君靜脈泵入抑制宮縮,地塞米松6 mg肌內(nèi)注射1次促胎肺成熟治療,并留置尿管。13+小時(shí)前無明顯誘因出現(xiàn)下腹部規(guī)律性脹痛伴呼吸困難、端坐呼吸,無法平臥,
[Abstract]:A 34-year-old patient was admitted to the hospital at 19:00 on June 13, 2015, for 31 6 weeks of gestation, 13 hours of lower abdominal distension and dyspnea. Patient plain menstruation, the last menstruation: November 2, 2014. There is no regular antenatal examination during pregnancy. No headache, palpitations, shortness of breath, itchy skin and edema of both legs. Privately charged plain vegetable likes to eat fat meat, eat greasy food for a long time, have symptoms of upper respiratory tract infection after 4 days of cold, manifested as runny, sore throat, no chills, fever, cough, etc. The symptoms were relieved after the treatment of oral medicine (specific details unknown). One day ago, the symptoms of frequent urination, urinal urgency, urinal pain, and not feeling the urine. The local hospital was given Litojun hydrochloride intravenous pump to suppress uterine contractions. Dexamethasone was injected intramuscularly for 6 mg to promote fetal lung maturation and indwelling urethral catheter. There was no obvious inducement to develop regular distending pain in lower abdomen with dyspnea 13 hours ago.
