【摘要】：目的單純性子癇前期與慢性高血壓并發(fā)子癇前期對母兒的影響。方法收集2009年1月至2015年6月于廣州醫(yī)科大學(xué)附屬第三醫(yī)院產(chǎn)科住院部分娩的、單胎、無合并其他基礎(chǔ)疾病的妊娠期高血壓疾病共596例臨床資料進(jìn)行回顧性分析,其中輕度子癇前期(MPE)101例、重度子癇前期(SPE)377例、慢性高血壓并發(fā)子癇前期(CHPE)84例、妊娠合并慢性高血壓(CHCP)34例。比較四組的一般情況、輔助檢查結(jié)果、分娩情況及母兒結(jié)局。結(jié)果與MPE組、SPE組相比,CHPE組的年齡偏大(33.58±5.28),經(jīng)產(chǎn)婦(69%)、雙親中至少一人患高血壓者(31%)等比例大于單純性子癇前期組。SPE組的尿酸值最高;CHPE組與SPE組的產(chǎn)前24 h尿蛋白差異無統(tǒng)計(jì)學(xué)意義,其剖宮產(chǎn)率高于MPE組及CHCP組,其終止妊娠時(shí)間早于MPE組和CHCP組。CHPE組與SPE組在新生兒體重、新生兒窒息率、Apgar評分等方面差異無統(tǒng)計(jì)學(xué)意義。CHPE組娩出活嬰數(shù)量最少,新生兒中男嬰所占比例大。結(jié)論與單純性子癇前期相比,慢性高血壓并發(fā)子癇前期孕婦的平均年齡大、高血壓家族史比例高,輔助檢查指標(biāo)以及母兒結(jié)局與重度子癇前期相近。
[Abstract]:Objective to study the effect of simple preeclampsia and chronic hypertension complicated with preeclampsia on mother and infant. Methods from January 2009 to June 2015, a total of 596 cases of pregnancy-induced hypertension complicating hypertension were retrospectively analyzed in the department of obstetrics and obstetrics of the third affiliated Hospital of Guangzhou Medical University. Among them, 101 cases of mild preeclampsia (MPE), 377 cases of severe preeclampsia (SPE), 84 cases of chronic hypertension complicated with pre-eclampsia (CHPE), 34 cases of pregnancy complicated with chronic hypertension (CHCP). The general condition, the results of the auxiliary examination, the delivery condition and the maternal and infant outcome were compared among the four groups. Results compared with MPE group and SPE group, the age of CHPE group was older (33.58 鹵5.28). The proportion of hypertensive patients with at least one parent (31%) in CHPE group was higher than that in simple preeclampsia group (31%). The uric acid value of SPE group was the highest. The rate of cesarean section in CHPE group and SPE group was higher than that in MPE group and CHCP group, and the termination time of pregnancy was earlier than that in MPE group and CHCP group. The neonatal weight and asphyxia rate in CHPE group and SPE group were higher than those in SPE group. There was no significant difference in Apgar score. In CHPE group, the number of live infants was the least, and the proportion of male infants was large. Conclusion compared with simple preeclampsia, the average age of pregnant women with chronic hypertension complicated with pre-eclampsia is higher, the proportion of hypertension family history is higher, the indexes of auxiliary examination and the maternal and infant outcome are similar to those of severe preeclampsia.
