【摘要】：目的探討不同控制性超促排卵(controlled ovarian hyperstimulation,COH)方案中血清及卵泡液(follicular fluid,FF)中血管內(nèi)皮生長因子(vascular endothelial growth factor,VEGF)與抑制素B(inhibin B,INHB)及COH結(jié)局的相關(guān)性。方法應(yīng)用酶聯(lián)免疫吸附方法分別檢測長方案組(A組,n=38)及非降調(diào)節(jié)方案組(B組,n=38)患者血清及FF中VEGF和INHB水平并行相關(guān)性分析。結(jié)果 1降調(diào)節(jié)足夠長時(shí)間(≥18 d)時(shí),血清VEGF水平顯著下降,且促性腺激素(Gn)啟動(dòng)日A組VEGF水平[(81.50±32.59)ng/L]明顯低于B組[(123.64±53.90)ng/L](P0.01)。2 Gn啟動(dòng)后A、B組血清VEGF水平呈上升趨勢,h CG注射日后A組繼續(xù)上升,而B組先下降后上升;且h CG注射次日A組VEGF水平[(165.69±38.25)ng/L]明顯高于B組[(136.49±39.19)ng/L](P=0.002);A、B組其它時(shí)間血清VEGF水平無統(tǒng)計(jì)學(xué)差異(P0.05)。3 Gn第5日血清VEGF水平與COH結(jié)局相關(guān)性最好(P0.01)。4 A、B組FF中VEGF水平隨卵泡直徑增大而升高,大、中卵泡VEGF水平無統(tǒng)計(jì)學(xué)差異(P0.05),但B組小卵泡VEGF水平[(889.86±268.95)ng/L]明顯高于A組[(728.45±291.88)ng/L](P=0.014);且大卵泡VEGF水平與COH結(jié)局顯著呈負(fù)相關(guān)。5 A、B組血清及FF中VEGF水平與INHB呈顯著負(fù)相關(guān)。結(jié)論 COH過程中,血清及大、中卵泡VEGF水平與卵巢反應(yīng)性及COH結(jié)局負(fù)相關(guān),并且可能對(duì)INHB分泌起抑制作用。
