【摘要】：目的通過檢測宮頸癌患者治療前外周血中細胞角蛋白(CK)19 mRNA和鱗狀細胞癌抗原(SCC-Ag)的表達,探討其對宮頸癌的診斷意義及與臨床病理的相關(guān)性。方法對100例宮頸癌患者行治療前外周血CK19 mRNA及SCC-Ag檢測,以40例健康婦女為對照,分析其作為診斷參考的特異性和敏感性,并對兩者與宮頸癌臨床病理特征的相關(guān)性進行分析。結(jié)果 CK19 mRNA、SCC-Ag檢測診斷宮頸癌的特異性均為97.5%。CK19 mRNA、SCC-Ag水平升高用于治療前診斷的敏感性分別為39%和57%,兩者差異顯著(P0.05)。CK19 mRNA表達升高與深肌層浸潤(P=0.031)、盆腔淋巴結(jié)轉(zhuǎn)移(P=0.027)相關(guān)。SCC-Ag水平升高與病理類型(P=0.005)、腫瘤大小(P=0.002)、深肌層浸潤(P=0.001)、盆腔淋巴結(jié)轉(zhuǎn)移(P=0.012)顯著相關(guān)。結(jié)論外周血CK19 mRNA和SCC-Ag均是宮頸癌微轉(zhuǎn)移有價值的腫瘤標志物;就宮頸鱗癌患者而言,SCC-Ag仍然是目前首選的腫瘤標志物。
[Abstract]:Objective to detect the expression of cytokeratin (CK) 19 mRNA and squamous cell carcinoma antigen (SCC-Ag) in peripheral blood of patients with cervical cancer before treatment, and to explore the diagnostic significance and clinicopathological significance of the expression of cytokeratin 19 mRNA and squamous cell carcinoma antigen (SCC-Ag). Methods Peripheral blood CK19 mRNA and SCC-Ag were detected in 100 patients with cervical cancer before treatment and 40 healthy women as control. The correlation between them and the clinicopathological features of cervical cancer was analyzed. Results the specificity of CK19 mRNA,SCC-Ag in the diagnosis of cervical cancer was 39% and 57%, respectively. There was significant difference between the two groups (P0.05). The increase of CK19 mRNA expression was associated with deep muscular invasion (P0.031), pelvic lymph node metastasis (P0. 027), and SCC-Ag level with pathological type (P0. 005) and tumor size (P0. 002). Deep muscular infiltration (P0. 001) and pelvic lymph node metastasis (P0. 012) were significantly correlated. Conclusion both CK19 mRNA and SCC-Ag in peripheral blood are valuable tumor markers for micrometastasis of cervical cancer and SCC-Ag is still the first choice for cervical squamous cell carcinoma.
