TSP-1、TSP-2、MVD與復(fù)發(fā)性流產(chǎn)的相關(guān)性研究

【摘要】:目的:通過研究復(fù)發(fā)性流產(chǎn)患者的絨毛、蛻膜組織中血小板反應(yīng)蛋白-1(TSP-1)、血小板反應(yīng)蛋白-2(TSP-2)表達(dá)及微血管密度(MVD)的大小情況,探討TSP-1、TSP-2與RSA的關(guān)系,為其病因?qū)W研究及治療提供新思路。方法:選擇2013年9月-2014年7在山西大醫(yī)院婦產(chǎn)科門診因復(fù)發(fā)性流產(chǎn)行人工流產(chǎn)的患者50例作為研究組,同時(shí)選擇同期既往有正常足月分娩史,而無不良生育史,本次正常妊娠并自愿行人工流產(chǎn)患者30例作為對照組,在人工流產(chǎn)的同時(shí)留取絨毛、蛻膜組織。采用免疫組化SP法檢測TSP-1、TSP-2在兩組絨毛及蛻膜組織中的表達(dá)水平(灰度值越低,表達(dá)水平越高)。選用CD34標(biāo)記血管內(nèi)皮細(xì)胞檢測兩組絨毛及蛻膜組織中微血管密度(MVD)值。研究結(jié)果用SPSS17.0統(tǒng)計(jì)軟件進(jìn)行分析,以α=0.05為檢驗(yàn)水準(zhǔn)。結(jié)果:1.絨毛組織中,研究組中TSP-1的的表達(dá)明顯高于對照組(TSP-1的灰度值顯著低于對照組)(P0.05),TSP-2的表達(dá)低于對照組(P0.05),TSP-1/TSP-2的比值(表達(dá))高于較對照組(P0.05),差異有統(tǒng)計(jì)學(xué)意義。在蛻膜組織中,研究組內(nèi)的TSP-1的表達(dá)明顯高于對照組(P0.05),TSP-2的表達(dá)顯著低于對照組,TSP-1/TSP-2比值(表達(dá))高于對照組(P0.05),差異有統(tǒng)計(jì)學(xué)意義。研究組中,絨毛組織內(nèi)TSP-1、TSP-2的表達(dá)明顯高于蛻膜組織(P0.05),TSP-1/TSP-2的(表達(dá))比值高于蛻膜組織(P0.05),差異有統(tǒng)計(jì)學(xué)意義。對照組中,絨毛組織內(nèi)TSP-1、TSP-2的表達(dá)高于蛻膜組織(P0.05),TSP-1/TSP-2的(表達(dá))比值低于蛻膜組織,差異有統(tǒng)計(jì)學(xué)意義。2.不論在絨毛組織中還是蛻膜組織中,研究組中MVD值均顯著高于對照組,但在絨毛組織中差異有統(tǒng)計(jì)學(xué)意義,而蛻膜組織中差異無統(tǒng)計(jì)學(xué)意義。在研究組中,絨毛組織中MVD值顯著低于蛻膜組織,差異有統(tǒng)計(jì)學(xué)意義。在對照組中,絨毛組織中MVD值高于蛻膜組織,差異有統(tǒng)計(jì)學(xué)意義。3.在絨毛組織中,TSP-1與TSP-2的表達(dá)呈正相關(guān),TSP-1,TSP-2的灰度值與MVD值均呈正相關(guān)(r分別為0.598、0.556,P0.05),即TSP-1、TSP-2的表達(dá)與MVD值呈負(fù)相關(guān)。在蛻膜組織中,TSP-1與TSP-2的灰度值呈負(fù)相關(guān)(r=-0.229,P0.05),即二者的表達(dá)呈負(fù)相關(guān);TSP-1的灰度值與CD34的表達(dá)MVD值呈正相關(guān)(r=0.360,P0.05),即TSP-1的表達(dá)與MVD值呈負(fù)相關(guān),而TSP-2的表達(dá)與MVD值二者相關(guān)性不明顯(r=0.017,P0.05)。結(jié)論:1.復(fù)發(fā)性流產(chǎn)患者的絨毛及蛻膜組織中,TSP-1呈高表達(dá)狀態(tài),TSP-2呈低表達(dá),TSP-1/TSP-2呈高表達(dá),推測TSP-1、TSP-2的表達(dá)異?赡苁菍(dǎo)致復(fù)發(fā)性流產(chǎn)發(fā)生的原因。2.研究組絨毛組織中MVD值明顯低于對照組,低于本組蛻膜組織中MVD值,而在正常對照組中,絨毛組織MVD值顯著高于蛻膜組織,提示絨毛組織中血管生成障礙是導(dǎo)致部分流產(chǎn)發(fā)生的主要原因。3.研究組絨毛組織中TSP-1的表達(dá)高于對照組,TSP-2的表達(dá)低于對照組,TSP-1/TSP-2的表達(dá)顯著高于對照組,而MVD值顯著低于對照組,推測局部TSP-1過度表達(dá)對血管生成障礙的影響可能更大。
[Abstract]:Objective: to investigate the expression of platelet-reactive protein-1 (TSP-1), platelet-reactive protein-2 (TSP-2) and microvessel density (MVD) in villi and decidua of patients with recurrent abortion. The relationship between TSP-2 and RSA provides a new idea for etiology research and treatment. Methods: from September 2013 to July 2014, 50 patients with recurrent abortion in Department of Gynecology and Obstetrics Clinic of Shanxi Great Hospital were selected as study group. 30 cases of normal pregnancy and spontaneous abortion were used as control group, villi and decidua tissue were retained simultaneously. The expression of TSP-1,TSP-2 in chorionic villi and decidua tissues was detected by immunohistochemical SP method (the lower the gray level, the higher the expression level). CD34 labeled vascular endothelial cells were used to detect the microvessel density (MVD) in chorionic villi and decidua. The results were analyzed by SPSS17.0 software, and the test level was 偽 = 0. 05. The result is 1: 1. In chorionic villi, the expression of TSP-1 in the study group was significantly higher than that in the control group (P 0.05), and the expression of TSP-2 in the study group was significantly lower than that in the control group (P0.05). The ratio of TSP-1/TSP-2 (expression) was higher than that of control group (P0.05), the difference was statistically significant. In decidual tissues, the expression of TSP-1 and TSP-2 in the study group was significantly higher than that in the control group (P0.05), and the TSP-1/TSP-2 ratio was significantly higher than that in the control group (P0.05). The difference is statistically significant. In the study group, the expression of TSP-1,TSP-2 in chorionic villi was significantly higher than that in decidua (P0.05), and the ratio of TSP-1/TSP-2 was higher than that in decidua (P0.05). In the control group, the expression of TSP-1,TSP-2 in chorionic villi was higher than that in decidual tissue (P0.05), and the ratio of TSP-1/TSP-2 (expression) was lower than that in decidua tissue, the difference was statistically significant. Both in chorionic villi and decidua tissues, MVD values in study group were significantly higher than those in control group, but there was significant difference in chorionic villus tissue, but no significant difference in decidua tissue. In the study group, MVD in chorionic villi was significantly lower than that in decidua tissue, and the difference was statistically significant. In the control group, the MVD value in chorionic villi was higher than that in decidua tissue, and the difference was statistically significant. In chorionic villi, the expression of TSP-1 and TSP-2 was positively correlated, and the gray value of TSP-1,TSP-2 was positively correlated with the value of MVD (r = 0.5980.556p0.05), that is, the expression of TSP-1,TSP-2 was negatively correlated with the value of MVD. In decidua tissue, the gray value of TSP-1 and TSP-2 was negatively correlated (r = -0.229, P 0.05), that is, the expression of TSP-1 and TSP-2 was negatively correlated. There was a positive correlation between the gray value of TSP-1 and the expression of MVD of CD34 (r = 0.360 0 P 0.05), that is, the expression of TSP-1 was negatively correlated with the value of MVD, but the expression of TSP-2 was not correlated with the value of MVD (r 0.017 P 0.05). Conclusion: 1. In the villus and decidua of recurrent abortion, the expression of TSP-1 was high, the expression of TSP-2 was low, and the expression of TSP-1/TSP-2 was high. We speculated that TSP-1, was expressed in patients with recurrent abortion. Abnormal expression of TSP-2 may be the cause of recurrent abortion. 2. 2. The MVD value of chorionic villi in the study group was significantly lower than that in the control group and the MVD value in decidua tissue, while in the normal control group, the MVD value of chorionic villi was significantly higher than that of decidua tissue. It is suggested that the main cause of partial abortion is vascular dysfunction in chorionic villi. 3. 3. The expression of TSP-1, TSP-2, TSP-1/TSP-2 and MVD in the chorionic villi of the study group were significantly higher than those in the control group. It is speculated that local overexpression of TSP-1 may have a greater effect on angiogenesis disorder.
