反復(fù)植入失敗患者移植日子宮內(nèi)膜蠕動(dòng)波的研究

【摘要】:目的:研究反復(fù)植入失敗患者移植日的子宮內(nèi)膜蠕動(dòng)波的情況,探討反復(fù)植入失敗患者是否存在子宮內(nèi)膜蠕動(dòng)波異常。 方法:本研究采用的是前瞻性巢式病例對照研究方式。經(jīng)ART治療的不孕女性的知情同意后,在移植日使用經(jīng)陰道B超采集其子宮內(nèi)膜蠕動(dòng)波的資料并于4倍于正常速度下兩名分析人員獨(dú)立分析頻率及類型。追蹤其妊娠結(jié)局后予以分組,妊娠結(jié)局陽性的女性根據(jù)其治療方案分別納入對應(yīng)的凍胚周期-妊娠組及鮮胚周期-妊娠組,納入組的病例數(shù)分別為50例和17例;反復(fù)植入失敗的女性相對應(yīng)的納入凍胚周期-RIF組及鮮胚周期-RIF組,獲得的病例數(shù)分別為32例和11例。比較分析RIF組與妊娠組的子宮內(nèi)膜蠕動(dòng)波的頻率及類型。 結(jié)果: 1.對于子宮內(nèi)膜蠕動(dòng)波的頻率,鮮胚周期合并凍胚周期的總周期中,RIF組明顯高于妊娠組,差異有顯著性(2.28±0.58vs.1.81±0.64P0.01);鮮胚周期-RIF組較鮮胚周期-妊娠組稍高,差異無顯著性(1.93±0.36vs.1.64±0.67,P=0.2);凍胚周期-RIF組明顯高于凍胚周期-妊娠組,差異有顯著性(2.40±0.59vs.1.88±0.62,P0.01)。 2.子宮內(nèi)膜蠕動(dòng)波頻率在RIF組中與移植周期數(shù)成正相關(guān)(r=0.34,P=0.028),在妊娠組中與移植周期數(shù)無明顯相關(guān)關(guān)系(r=0.034,P=0.78)。 3.對于不同類型的子宮內(nèi)膜蠕動(dòng)波的出現(xiàn)率,無論是鮮胚周期還是凍胚周期中,RIF組與妊娠組差異均無顯著性(P0.05)。 4.對于不同類型的子宮內(nèi)膜蠕動(dòng)波的構(gòu)成比,無論是鮮胚周期還是凍胚周期中,RIF組與妊娠組差異均無顯著性(P0.05)。 結(jié)論: 1.子宮內(nèi)膜蠕動(dòng)波頻率增高可能是影響反復(fù)植入失敗患者胚胎著床的原因之一。 2.反復(fù)植入失敗患者子宮內(nèi)膜蠕動(dòng)波頻率與移植周期數(shù)成正相關(guān)。
[Abstract]:Aim: to study the endometrial peristalsis in patients with failed implantation on the day of transplantation, and to investigate whether there is abnormal endometrial peristalsis in patients with repeated failed implantation. Methods: a prospective nested case-control study was conducted. After informed consent of infertile women treated with ART, the data of endometrial peristalsis were collected by transvaginal ultrasound on the day of transplantation, and the frequency and type of endometrial peristalsis were independently analyzed by two analysts at 4 times the normal speed. The women with positive pregnancy outcome were included in the corresponding frozen embryo cycle pregnancy group and the fresh embryo cycle pregnancy group according to their treatment plan. The number of cases in the group were 50 and 17 respectively. The number of women who failed repeatedly in the frozen embryo cycle-RIF group and the fresh embryo cycle-RIF group were 32 and 11, respectively. The frequency and type of endometrial peristalsis in RIF group and pregnancy group were compared. Results: 1. For the frequency of endometrial peristalsis, the frequency of peristaltic wave in RIF group was significantly higher than that in pregnancy group (2.28 鹵0.58vs.1.81 鹵0.64P0.01).
