【摘要】：目的:探討hCG注射日子宮內(nèi)膜上段和中段2個(gè)測(cè)量點(diǎn)的厚度之差與體外受精-胚胎移植(IVF-ET)妊娠結(jié)局之間的相關(guān)關(guān)系。方法:對(duì)初次接受IVF-ET治療的256例患者,在hCG注射日行陰道超聲測(cè)量子宮內(nèi)膜最大厚度、上段厚度(距宮底10 mm處)、中段厚度(距宮底20 mm處)、上中段內(nèi)膜差(ETD,即上段厚度-中段厚度)。根據(jù)ETD將患者分為4組:A組(ETD0 mm);B組(ETD 0~2.0 mm);C組(ETD 2.1~4.0 mm);D組(ETD4.0 mm),然后以年齡35歲為界將患者分為2個(gè)亞組:≥35歲和35歲,比較各組的臨床妊娠率、胚胎著床率和繼續(xù)妊娠率。結(jié)果:年齡35歲患者總的臨床妊娠率為59.16%,4組胚胎著床率分別是39.3%、38.3%、51.2%和27.5%,4組之間有統(tǒng)計(jì)學(xué)差異(P=0.038 6)。4組的臨床妊娠率為59.1%、60.6%、67.7%和45.0%(P=0.525);繼續(xù)妊娠率為52.3%、50.0%、54.1%和30.0%,C組高于其余各組,但無統(tǒng)計(jì)學(xué)差異(P=0.296)。年齡≥35歲患者臨床妊娠率為43.8%,較35歲者低,且不同ETD組間各種觀察數(shù)據(jù)無統(tǒng)計(jì)學(xué)差異(P0.05)。結(jié)論:年輕患者ETD在2.1~4.0 mm范圍時(shí)可能預(yù)示更佳的妊娠結(jié)局。
