【摘要】：目的探討高危型人乳頭瘤病毒(HPV)分型檢測對宮頸癌及癌前病變篩查的意義,為臨床宮頸病變的診治提供依據(jù)。方法選擇2013年1月-2015年6月嘉善縣第一人民醫(yī)院婦科門診就診的并自愿進行HPV篩查的7 521例患者作為研究對象,所有研究對象均進行高危型HPV分型檢測,對于HPV檢測陽性或者有臨床癥狀需要進行宮頸液基細胞學檢查,HPV陽性或者宮頸液基細胞學檢查陽性,或者臨床癥狀需要行陰道鏡檢查者給予陰道鏡檢查,根據(jù)陰道鏡檢查情況,必要時行宮頸活檢行病理學檢查。觀察HPV篩查患者中高危型HPV的感染情況,不同年齡段中HPV感染情況,不同宮頸病變中高危型HPV的感染情況。結果 7 521例進行HPV分型篩查的患者中,HPV陽性患者1 128例,HPV感染率為15.0%;HPV陽性患者的年齡高于HPV陰性患者(P0.05),不同高危分型HPV感染中,感染率最高的為HPV16型。HPV感染隨著年齡的增加HPV感染率也不斷增加,20~29歲年齡段患者HPV的感染率最低,為13.0%,60歲以上患者的HPV感染率最高,為36.9%,各年齡段之間HPV感染率之間比較,差異有統(tǒng)計學意義(P0.05)。325例行宮頸活檢病理檢查的患者中,檢出CINⅠ115例、CINⅡ47例、CINⅢ80例、宮頸癌10例。不同宮頸病變中各種高危型HPV的感染情況之間比較,差異有統(tǒng)計學意義(P0.05),其中HPV16分型、HPV52分型、HPV56分型和HPV58分型宮頸病變發(fā)生的比例比較高。結論宮頸高危型HPV分型檢測對宮頸癌及宮頸癌前病變的診斷具有重要意義,HPV16分型、HPV52分型、HPV56分型和HPV58分型宮頸癌和宮頸癌前病變的發(fā)生率比較高,在有條件的醫(yī)院,宮頸高危型HPV分型檢查可以作用宮頸癌和宮頸癌前病變的篩查手段。
[Abstract]:Objective to investigate the significance of high risk human papillomavirus (HPV) (HPV) typing in screening cervical cancer and precancerous lesions, and to provide evidence for clinical diagnosis and treatment of cervical lesions. Methods from January 2013 to June 2015, 7,521 patients from gynecological outpatient clinic of Jiashan first people's Hospital and voluntary HPV screening were selected as the study objects. All the subjects were tested for high-risk HPV typing. For those whose HPV is positive or who has clinical symptoms that require cervical liquid-based cytology, HPV positive or cervical liquid-based cytology, or those whose clinical symptoms need to be examined by colposcopy, According to colposcopy, cervical biopsy was performed as necessary for pathological examination. The infection of high risk HPV in HPV screening patients, HPV infection in different age groups and high risk HPV infection in different cervical lesions were observed. Results among the 7 521 patients with HPV typing, 1 128 were HPV positive, and the HPV infection rate was 15.0. The age of HPV positive patients was higher than that of HPV negative patients (P0.05). The infection rate of HPV16 type was the highest among different high risk HPV types.
