【摘要】：目的探討COOK球囊用于高齡孕婦足月引產(chǎn)促宮頸成熟的有效性和安全性。方法回顧分析江門市新會(huì)區(qū)婦幼保健計(jì)劃生育服務(wù)中心、廣州市南方醫(yī)科大學(xué)南方醫(yī)院2015年1月至2016年3月50例應(yīng)用COOK球囊引產(chǎn)的高齡孕婦資料,同時(shí)以50例同期COOK球囊引產(chǎn)的非高齡孕婦資料為對(duì)照。統(tǒng)計(jì)兩組年齡、孕周、孕產(chǎn)次、引產(chǎn)指征、引產(chǎn)前后宮頸Bishop評(píng)分、產(chǎn)程時(shí)間、剖宮產(chǎn)指征及分娩結(jié)局,比較兩組間差異。結(jié)果兩組孕婦除年齡外,引產(chǎn)孕周、孕次、產(chǎn)次比較均無(wú)統(tǒng)計(jì)學(xué)意義。妊娠期糖尿病在高齡組孕婦中所占的比例顯著高于非高齡組。單純宮頸不成熟作為引產(chǎn)指征,非高齡組顯著高于高齡組孕婦(P0.05)。高齡組孕婦應(yīng)用球囊前后宮頸Bishop評(píng)分分別為(3.85±0.27)分、(5.76±1.33)分,應(yīng)用球囊后顯效及有效率為84.2%。非高齡組孕婦應(yīng)用球囊前后宮頸Bishop評(píng)分分別為(4.27±0.48)分、(6.23±1.23)分,應(yīng)用球囊后顯效及有效率為87.5%。兩組前后宮頸評(píng)分、有效率比較均無(wú)統(tǒng)計(jì)學(xué)差異。兩組間第一產(chǎn)程、第二產(chǎn)程時(shí)間相比,高齡組長(zhǎng)于非高齡組,有統(tǒng)計(jì)學(xué)意義。兩組不良反應(yīng)、剖宮產(chǎn)指征及分娩結(jié)局比較無(wú)統(tǒng)計(jì)學(xué)差異。結(jié)論 COOK球囊促宮頸成熟在高齡孕婦中的有效性和安全性與非高齡孕婦相當(dāng),對(duì)高齡孕婦是一種實(shí)用且安全的臨床引產(chǎn)方法。
[Abstract]:Objective to evaluate the efficacy and safety of COOK balloon in term induction of cervix maturation. Methods the data of 50 elderly pregnant women who were induced labor by COOK balloon from January 2015 to March 2016 were analyzed retrospectively in the Maternal and Child Health and Family Planning Service Center of Xinhui District of Jiangmen City and the Southern Hospital of Southern Medical University of Guangzhou from January 2015 to March 2016. The data of 50 non-elderly pregnant women with COOK balloon induced labor at the same time were compared. The age, gestational weeks, times of pregnancy, indication of induced labor, Bishop score of cervix, time of labor, indication of cesarean section and the outcome of delivery were compared between the two groups. Results there was no significant difference between the two groups except age, gestational week, pregnancy and delivery. The proportion of gestational diabetes mellitus in the elderly group was significantly higher than that in the non-elderly group. Simple cervical immaturity as an indication of induced labor, the non-elderly group was significantly higher than the elderly group (P0.05). In the elderly group, the cervical Bishop scores were (3.85 鹵0.27) and (5.76 鹵1.33) before and after balloon application, respectively. The effective rate was 84.2%. The cervical Bishop scores were (4.27 鹵0.48) and (6.23 鹵1.23) before and after balloon application, respectively. The effective rate was 87.5%. There was no significant difference in the effective rate between the two groups before and after cervix score. The time of the first stage of labor and the second stage of labor between the two groups were significantly higher than that in the non-elderly group. There was no significant difference in adverse reactions, indications of cesarean section and delivery outcome between the two groups. Conclusion the efficacy and safety of COOK balloon in promoting cervix maturation in elderly pregnant women is similar to that in non-elderly pregnant women. It is a practical and safe clinical induction method for elderly women.
