【摘要】：目的:應(yīng)用Tei指數(shù)探討母體高血糖環(huán)境對胎兒及其出生后整體心功能的影響。方法:選取孕28~41+6周胎兒205例(正常組120例,妊娠期糖尿病組85例),足月產(chǎn)1~7天新生兒90例(正常組30例,血糖控制良好組30例,血糖控制不良組30例),足月產(chǎn)2~3個月嬰兒75例(正常組30例,血糖控制良好組25例,血糖控制不良組20例)。測量左、右心室Tei指數(shù)。結(jié)果:與正常妊娠組(左室:0.41±0.17、右室:0.41±0.16)比較,血糖控制良好組胎兒的心室Tei指數(shù)(左室:0.44±0.12,右室:0.44±0.11)無差異(P0.05),血糖控制不良組(左室:0.50±0.17,右室:0.50±0.10)均增高(P0.05)。與正常新生兒組(左室:0.40±0.14,右室:0.26±0.11)比較,血糖控制良好組(左室:0.43±0.13,右室:0.28±0.14)無差異(P0.05),血糖控制不良組(左室:0.46±0.16,右室:0.31±0.12)均增高(P0.05)。與正常嬰兒組(左室:0.39±0.13,右室:0.25±0.14)比較,血糖控制良好組(左室:0.40±0.12,右室:0.25±0.17)無差異(P0.05),血糖控制不良組(左室:0.43±0.16,右室:0.29±0.12)稍高,差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:GDM血糖控制不良胎兒心功能受損,新生兒、嬰兒心功能仍低于正常水平。
[Abstract]:Objective: to investigate the effect of maternal hyperglycemia on fetal and postnatal cardiac function by Tei index. Methods: 205 fetuses (120 normal, 85 gestational diabetes) and 90 newborns (30 normal and 30 well controlled) were selected. There were 30 cases in poor blood glucose control group, 75 cases in term birth 2 ~ 3 months group (30 cases in normal group, 25 cases in good blood glucose control group and 20 cases in poor blood glucose control group). Tei index of left and right ventricle was measured. Results: compared with the normal pregnancy group (left ventricular: 0.41 鹵0.17, right ventricle: 0.41 鹵0.16), the ventricular Tei index (left ventricular: 0.44 鹵0.12, right ventricle: 0.44 鹵0.11) of the fetus with good blood glucose control group had no difference (P0.05), while that of the poor blood glucose control group (left ventricle: 0.50 鹵0.17, right ventricle: 0.50 鹵0.10) was higher (P0.05). Compared with the normal neonates group (left ventricular: 0.40 鹵0.14, right ventricle: 0.26 鹵0.11), there was no difference between the two groups (left ventricle: 0.43 鹵0.13, right ventricle: 0.28 鹵0.14) (P0.05), but the blood glucose control group (left ventricle: 0.46 鹵0.16, right ventricle: 0.31 鹵0.12) were higher than those in the control group (P0.05). Compared with normal infant group (left ventricular: 0.39 鹵0.13, right ventricle: 0.25 鹵0.14), there was no significant difference in blood glucose control group (left ventricle: 0.40 鹵0.12, right ventricle: 0.25 鹵0.17) (P0.05), but poor blood glucose control group (left ventricle: 0.43 鹵0.16, right ventricle: 0.29 鹵0.12), the difference was statistically significant (P0.05). Conclusion: the fetal heart function of GDM is impaired and the heart function of the newborn is still lower than the normal level.
