【摘要】：目的:研究宮頸內口-子宮下段壓迫縫合法對于兇險性前置胎盤伴胎盤植入導致的術中子宮出血的治療效果,以及實施該手術對于患者的遠期影響。方法:選擇2013年1月1日至2015年12月31日于廈門大學附屬第一醫(yī)院進行剖宮產的兇險性前置胎盤伴胎盤植入的患者,對其中應用宮頸內口-子宮下段壓迫縫合法術中處理的16例患者的臨床資料進行回顧性分析。結果:16例患者的平均分娩孕周35±3周,所有新生兒預后良好。術中平均出血量1300±499 ml。所有患者術中未出現(xiàn)周圍器官損傷等手術并發(fā)癥。無一例子宮切除。術后平均住院日5.1±1.6天,無一例轉重癥監(jiān)護病房。術后產褥感染2例,經過加強抗感染治療后痊愈。術后3個月及半年隨訪彩超檢查,1例懷疑剖宮產瘢痕憩室形成。結論:宮頸內口-子宮下段壓迫縫合法對于兇險性前置胎盤伴胎盤植入導致的術中子宮出血的治療效果較好。
[Abstract]:Objective: to study the effect of cervical internal oro-uterine compression suture for intraoperative uterine bleeding caused by severe placenta previa with placenta accreta and its long-term effect. Methods: from January 1, 2013 to December 31, 2015, patients with dangerous placenta previa with placenta accreta were performed caesarean section at the first affiliated Hospital of Xiamen University. The clinical data of 16 patients treated with cervical internal oro-uterine compression suture were retrospectively analyzed. Results the average gestational age of 16 cases was 35 鹵3 weeks, and the prognosis of all newborns was good. Mean intraoperative bleeding 1300 鹵499 ml. No complications such as peripheral organ injury were found in all patients. No hysterectomy was found. The average postoperative hospital stay was 5.1 鹵1.6 days, and none of the patients were transferred to intensive care unit. Two cases of puerperal infection were cured after intensive anti-infection therapy. After 3 months and half a year follow-up, color Doppler ultrasonography was performed in 1 case with suspected cicatricial diverticulum formation in cesarean section. Conclusion: the treatment of uterine bleeding caused by placenta previa and placenta accreta is effective.
