【摘要】：【目的】探討血清PTX3與子癇前期的關(guān)系，以及與子癇前期患者并發(fā)腎功能損害的相關(guān)性�！痉椒ā窟x取蘇州大學(xué)附屬第一醫(yī)院2013年9月至2014年2月于產(chǎn)科病房住院孕產(chǎn)婦75例，包括輕度子癇前期組（MPE組）25例，，重度子癇前期組（SPE組）25例，同期健康的孕婦25例作為對(duì)照組，采用酶聯(lián)免疫吸附法測(cè)定各組實(shí)驗(yàn)對(duì)象血清五聚素3的濃度，同時(shí)進(jìn)行超敏C反應(yīng)蛋白（hsCRP）、平均動(dòng)脈壓、24小時(shí)尿蛋白定量、中性粒細(xì)胞比率、尿酸、肌酐、尿素氮等各臨床指標(biāo)檢測(cè)�！窘Y(jié)果】（1）對(duì)照組、MPE組、SPE組三組血清PTX3值的水平分別為：（4.51±1.45）ng/ml、（7.75±1.41）ng/ml、（9.81±2.47）ng/ml，MPE組、SPE組孕婦血清中PTX3水平均高于對(duì)照組（P＜0.05），其中SPE組高于正常對(duì)照組（P＜0.01），SPE組高于MPE組（P＜0.05），且差異均具有統(tǒng)計(jì)學(xué)意義（P＜0.05）。（2）三組孕婦血清中PTX3水平與平均動(dòng)脈壓、中性粒細(xì)胞比率，24小時(shí)尿蛋白、及腎功能指標(biāo)尿酸、肌酐、尿素氮水平呈正相關(guān)關(guān)系（0＜r＜1，P＜0.05）；三組孕婦血清中hsCRP的表達(dá)水平與平均動(dòng)脈壓、中性粒細(xì)胞比率呈正相關(guān)關(guān)系（0＜r＜1，P＜0.05），而與24小時(shí)尿蛋白，及腎功能指標(biāo)尿酸、肌酐、尿素氮無(wú)直線相關(guān)關(guān)系（P＞0.05），兩者均與BMI、孕周無(wú)直線相關(guān)關(guān)系（P＞0.05）。（3）將試驗(yàn)所得血清PTX3、hsCRP濃度進(jìn)行ROC曲線分析，可見(jiàn)PTX3曲線下面積達(dá)0.905，hsCRP曲線下面積達(dá)0.821，提示PTX3對(duì)子癇前期疾病的診斷具有一定的準(zhǔn)確性，且價(jià)值較hsCRP高�！窘Y(jié)論】PTX3不僅與子癇前期的發(fā)生有關(guān)，且其表達(dá)水平與疾病的嚴(yán)重程度相關(guān)，亦可作為子癇前期腎功能損害的監(jiān)測(cè)指標(biāo)，且對(duì)子癇前期診斷的價(jià)值較hsCRP高。
[Abstract]:[objective] to investigate the relationship between serum PTX3 and preeclampsia, and the correlation between serum PTX3 and preeclampsia complicated with renal function damage. [methods] 75 pregnant women in obstetrical wards from September 2013 to February 2014 were selected from the first affiliated Hospital of Suzhou University. There were 25 cases of mild preeclampsia group (MPE group), 25 cases of severe preeclampsia group (SPE group) and 25 cases of healthy pregnant women as control group. At the same time, 24 hours urinary protein quantification, neutrophil ratio, uric acid, creatinine were measured by (hsCRP), mean arterial pressure. [results] (1) the serum PTX3 levels in the control group were (4.51 鹵1.45) ng/ml, (7.75 鹵1.41) ng/ml, (9.81 鹵2.47) ng/ml,MPE were higher than those in the control group (P < 0. 05), and the serum PTX3 level in the SPE group was higher than that in the normal control group (P < 0. 05). The serum PTX3 level and mean arterial pressure in the three groups were significantly higher than those in the MPE group (P < 0. 01), and the difference was statistically significant (P < 0. 05). There was a positive correlation between urinary protein in 24 hours of neutrophil ratio, uric acid, creatinine and urea nitrogen levels (0 < r < 1, P < 0. 05), and the expression of hsCRP in serum of the three groups was correlated with the mean arterial pressure. There was a positive correlation between neutrophil ratio (0 < r < 1g P < 0.05) and 24 hours urine protein, uric acid, creatinine, renal function index. There was no linear correlation between urea nitrogen and BMI, (P > 0. 05). There was no linear correlation between urea nitrogen and BMI, during gestational age (P > 0. 05). (3). The serum PTX3,hsCRP concentration was analyzed by ROC curve. It can be seen that the area under the PTX3 curve is 0.821, suggesting that PTX3 has a certain accuracy in the diagnosis of pre-eclampsia disease and its value is higher than that of hsCRP. [conclusion] PTX3 is not only related to the occurrence of pre-eclampsia, but also to the diagnosis of pre-eclampsia. The expression level was correlated with the severity of the disease and could also be used as a monitoring index for renal damage in preeclampsia. The diagnostic value of preeclampsia was higher than that of hsCRP.
