【摘要】：目的探討外源性病原菌在盆腔膿腫中的致病作用,為臨床治療盆腔膿腫提供參考。方法選取2014年1月-2016年6月盆腔膿腫住院患者105例為研究組,并選取同期住院的非感染性良性附件包塊患者107例為對照組;回顧性分析兩組患者的病歷資料,采集兩組患者的宮頸分泌物,分別檢測沙眼衣原體、人型支原體和解脲支原體;并做淋病奈瑟菌培養(yǎng);根據(jù)B超結(jié)果記錄兩組病例盆腔包塊直徑。結(jié)果研究組與對照組在避孕套使用方面差異有統(tǒng)計學意義(14.29%vs 41.12%,P0.01),在宮內(nèi)節(jié)育器放置方面差異有統(tǒng)計學意義(37.14%vs 9.35%,P0.01);兩組沙眼衣原體和人型支原體陽性率比較,分別為22.34%vs 5.61%和31.52%vs 9.80%,差異有統(tǒng)計學意義(P0.01);解脲脲原體在兩組中的感染率均最高(54.35%vs 55.88%),但兩組間差異無統(tǒng)計學意義;淋球菌感染率較低(2.56%vs 0.00%),兩組比較差異無統(tǒng)計學意義。結(jié)論使用安全套能降低盆腔炎性疾病的患病率,而使用宮內(nèi)節(jié)育器的婦女更容易感染盆腔炎性疾病;沙眼衣原體是盆腔膿腫的主要致病菌;人型支原體陽性率高于對照組的結(jié)果表明人型支原體也可能是盆腔炎性疾病的致病菌之一;兩組中的解脲脲原體均呈現(xiàn)高陽性率;由于解脲脲原體既是致病菌也是共生菌,單純的解脲脲原體陽性并不具有明確的臨床意義。
[Abstract]:Objective to investigate the pathogenicity of exogenous pathogens in pelvic abscess and to provide reference for clinical treatment of pelvic abscess. Methods from January 2014 to June 2016, 105 patients with pelvic abscess were selected as study group and 107 patients with noninfectious benign adnexal mass as control group. The cervical secretions of the two groups were collected, chlamydia trachomatis, Mycoplasma hominis and Ureaplasma Urealyticum were detected, and Neisseria gonorrhoeae was cultured. Results there were significant differences in condom use between the study group and the control group (14.29%vs 41.12g / P0.01) and in the placement of intrauterine device (37.14%vs 9.35g / P0.01), and the positive rates of chlamydia trachomatis and mycoplasma hominis were compared between the two groups. The infection rate of Ureaplasma Urealyticum in the two groups was the highest (54.35%vs 55.88%), but there was no significant difference between the two groups, and the infection rate of Neisseria gonorrhoeae was lower (2.56%vs 0.00%), there was no significant difference between the two groups. Conclusion condom use can reduce the prevalence of pelvic inflammatory diseases, while women who use intrauterine devices are more likely to be infected with pelvic inflammatory diseases, Chlamydia trachomatis is the main pathogen of pelvic abscess. The positive rate of Ureaplasma hominis is higher than that of the control group. The results indicate that Mycoplasma hominis may also be one of the pathogens of pelvic inflammatory disease. Ureaplasma Urealyticum showed a high positive rate in both groups. Because Ureaplasma Urealyticum is both a pathogen and a symbiotic bacterium, Simple Ureaplasma Urealyticum positive does not have clear clinical significance.
