【摘要】：目的了解不同時(shí)期河南省HIV暴露嬰幼兒母嬰傳播及生存情況,分析母嬰傳播的有關(guān)因素。方法通過預(yù)防艾滋病、梅毒及乙肝母嬰傳播信息系統(tǒng),收集不同工作時(shí)期HIV感染孕產(chǎn)婦及所分娩嬰兒的有關(guān)干預(yù)信息,包括孕產(chǎn)婦圍生期母嬰阻斷服務(wù)、嬰幼兒HIV感染及生存狀況等,采用Logistic回歸對(duì)影響HIV母嬰傳播的有關(guān)因素進(jìn)行分析。結(jié)果截止2014年12月底分娩的1 673例嬰幼兒中,存活滿18月齡嬰兒共1 515人(90.56%),其中檢測陽性嬰兒64人(3.83%),檢測陰性嬰兒1 451人(86.73%),已經(jīng)死亡的嬰兒101人(6.04%),失訪嬰兒57人(3.40%),嬰兒累計(jì)死亡率為6.24%,存活嬰兒累計(jì)母嬰傳播率為4.22%,校正累計(jì)艾滋病母嬰傳播率6.25%。產(chǎn)婦孕早期接受PMTCT服務(wù)(OR=0.23,95%CI:0.07~0.72),產(chǎn)婦及嬰兒藥物阻斷(OR=0.46,95%CI:0.23~0.95),嬰幼兒采人工喂養(yǎng)(0R=0.09,95%CI:0.02~0.35)是艾滋病母嬰傳播的保護(hù)因素,產(chǎn)婦分娩過程不安全助產(chǎn)(0R=3.52,95%CI:1.49~8.27)是其危險(xiǎn)因素。結(jié)論艾滋病暴露嬰幼兒的母嬰傳播率及死亡率均較高,應(yīng)進(jìn)一步加強(qiáng)孕產(chǎn)婦及嬰兒的隨訪管理,同時(shí)進(jìn)一步完善預(yù)防艾滋病母嬰傳播的綜合干預(yù)措施。
[Abstract]:Objective to investigate the status of mother-to-child transmission and survival of infants exposed to HIV in Henan Province in different periods, and to analyze the related factors of mother-to-child transmission. Methods through the information system for prevention of HIV / AIDS, syphilis and hepatitis B transmission from mother to child, the information about the intervention of pregnant women infected with HIV and their babies delivered in different working periods was collected, including the interruption of maternal and child delivery services during perinatal period. HIV infection and survival status of infants were analyzed by Logistic regression. Results of the 1,673 infants and children who gave birth at the end of December 2014, A total of 1,515 infants (90.56%) survived over 18 months of age, of whom 64 (3.83%) were tested positive, 1,451 (86.73%) were negative, 101 (6.04%) were dead, 57 (3.40%) were lost, the cumulative infant mortality rate was 6.24%, and the cumulative mother to child transmission of surviving infants was 6.24%. The rate was 4.22. The cumulative rate of mother-to-child transmission of AIDS was 6.25. PMTCT service (OR=0.23,95%CI:0.07~0.72), drug blocking (OR=0.46,95%CI:0.23~0.95), artificial feeding (0RR0. 09% CI: 0. 02% 0. 35) were the protective factors for mother-to-child transmission of HIV. Unsafe delivery attended delivery (0 RV 3. 52% 95 CI: 1. 498.27) was the risk factor. Conclusion the rate of mother-to-child transmission and mortality of infants exposed to AIDS are high. The follow-up management of pregnant women and infants should be further strengthened and the comprehensive intervention measures for prevention of mother-to-child transmission of AIDS should be further improved.
