【摘要】：目的:研究硬膜外分娩鎮(zhèn)痛與降低產(chǎn)后抑郁癥的發(fā)病風(fēng)險(xiǎn)是否相關(guān)。方法:根據(jù)納入排除標(biāo)準(zhǔn)及自愿參加本項(xiàng)研究原則,選擇符合條件的初產(chǎn)婦212人,按照是否自愿選擇硬膜外分娩鎮(zhèn)痛將其分為兩組,其中非鎮(zhèn)痛組113人,鎮(zhèn)痛組99人。記錄兩組初產(chǎn)婦不同時(shí)間點(diǎn)的數(shù)字疼痛評(píng)分(NRS)、舒適滿意度等圍產(chǎn)期相關(guān)變量,并于產(chǎn)后第3天和第42天分別行愛丁堡產(chǎn)后抑郁量表評(píng)分(EPDS)調(diào)查,記錄產(chǎn)后抑郁癥(PPD)發(fā)生情況。結(jié)果:最終有35名初產(chǎn)婦由于各種原因沒有完成整項(xiàng)研究,故有177人被包含在最終的數(shù)據(jù)分析中,其中非鎮(zhèn)痛組92人,鎮(zhèn)痛組85人。產(chǎn)后抑郁癥的發(fā)生率在接受硬膜外鎮(zhèn)痛組和非鎮(zhèn)痛組中分別為14.12%(85人有12人)和29.35%(92人有27人)(P=0.015)。使用硬膜外分娩鎮(zhèn)痛與產(chǎn)后抑郁癥的風(fēng)險(xiǎn)降低二者相關(guān)(OR 0.396,95%CI,0.185-0.844,P0.05),且產(chǎn)后3天和42天的EPDS評(píng)分之間存有相關(guān)性(Spearman相關(guān)系數(shù)=0.543,P0.001)。結(jié)論:硬膜外分娩鎮(zhèn)痛與降低產(chǎn)后抑郁癥的發(fā)生確實(shí)相關(guān),但仍需進(jìn)一步擴(kuò)大樣本量證實(shí)二者關(guān)聯(lián)的相關(guān)具體原因。
[Abstract]:Objective: to study whether epidural labor analgesia is associated with reducing the risk of postpartum depression. Methods: according to the principle of exclusion and voluntary participation, 212 primipara were selected and divided into two groups according to their voluntary choice of epidural labor analgesia, including 113 patients in non-analgesia group and 99 patients in analgesia group. Perinatal variables such as (NRS), comfort satisfaction were recorded at different time points of puerpera in two groups. (EPDS) survey was performed on the 3rd and 42nd postpartum, respectively, to record the incidence of (PPD) in postpartum depression. Results: the final 35 primipara did not complete the whole study for various reasons, so 177 were included in the final data analysis, including 92 patients in non-analgesic group and 85 in analgesia group. The incidence of postpartum depression was 14.12% in epidural analgesia group and 29.35% in non-analgesia group (P 0.015). Epidural labor analgesia was associated with a reduced risk of postpartum depression (OR 0.396C95), and there was a correlation between the EPDS scores of 3 and 42 days postpartum (Spearman correlation coefficient 0.543p0.001). Conclusion: epidural labor analgesia is associated with the reduction of postpartum depression, but it is still necessary to further expand the sample size to confirm the specific causes of the correlation between epidural analgesia and postpartum depression.
