【摘要】：目的對(duì)妊娠期糖尿病(GDM)患者晚孕期血脂水平變化與新生兒出生體重的相關(guān)性進(jìn)行研究。方法以2013年3月至2013年4月在湖北省天門市第一人民醫(yī)院就診的120例行孕期檢查的患者為研究對(duì)象,其中有43例在孕檢過程中發(fā)現(xiàn)糖尿病,作為觀察組,剩余77例為正常妊娠婦女,作為對(duì)照組,對(duì)比分析兩組患者孕晚期血脂水平與新生兒體重的相關(guān)性。結(jié)果觀察組TG、LDL-C、apoB水平明顯高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);觀察組LDL-C/HDL-C比值明顯高于對(duì)照組,apoA1/apoB比值明顯低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);GDM孕婦新生兒出生體重相比高于正常妊娠孕婦,差異有統(tǒng)計(jì)學(xué)意義(P0.05);新生兒出生體重與孕前BMI、孕周及孕晚期TG呈明顯正相關(guān)(P0.05),而與孕晚期脂蛋白水平呈明顯負(fù)相關(guān)(P0.05);血脂異常的GDM患者產(chǎn)后出血、早產(chǎn)兒、新生兒窒息、巨大兒和胎兒窘迫的發(fā)生率高于血脂正常的GDM患者,差異有統(tǒng)計(jì)學(xué)意義(P0.05或P0.01)。結(jié)論妊娠晚期孕婦均存在著不同程度的高脂血癥,新生兒出生體重與孕晚期TG水平呈明顯正相關(guān),GDM孕婦脂代謝紊亂較嚴(yán)重,因此加強(qiáng)GDM孕婦孕期飲食指導(dǎo)及血脂監(jiān)測有重要的臨床意義。
[Abstract]:Objective To study the relationship between the changes of blood lipid levels in late pregnancy and the birth weight of newborns in patients with gestational diabetes mellitus (GDM). Results The levels of TG, LDL-C and apoB in the observation group were significantly higher than those in the control group (P 0.05). The LDL-C/HDL-C ratio in the observation group was significantly higher than that in the control group, and the apoA1/apoB ratio was significantly lower than that in the control group. The birth weight of GDM pregnant women was significantly higher than that of normal pregnant women (P 0.05); the birth weight of GDM pregnant women was positively correlated with BMI before pregnancy, TG during pregnancy and late pregnancy (P 0.05), but negatively correlated with lipoprotein level in late pregnancy (P 0.05); and the birth weight of GDM patients with dyslipidemia was positively correlated with BMI before pregnancy, TG during pregnancy and late pregnancy (P 0.05). The incidence of hemorrhage, premature infant, neonatal asphyxia, macrosomia and fetal distress was higher than that of GDM patients with normal blood lipid (P 0.05 or P 0.01). Therefore, it is of great clinical significance to strengthen the dietary guidance and blood lipid monitoring of GDM pregnant women during pregnancy.
