【摘要】：目的探討乙肝疫苗聯(lián)合乙肝免疫球蛋白阻斷乙肝母嬰垂直傳播的臨床療效及影響因素。方法選取2012年6月—2014年6月江山市人民醫(yī)院收治的HBsAg陽性孕婦及其所產(chǎn)新生兒,各80例。新生兒出生后24 h注射乙肝免疫球蛋白(HBIG),并按"0-1-6"程序進行乙肝疫苗接種。檢測新生兒的HBsAg和HBs Ab,比較不同喂養(yǎng)方式、分娩方式、產(chǎn)婦"大三陽"http://www.cnki.com.cn/"小三陽"情況及產(chǎn)婦孕前是否按程序接種HBIG新生兒的HBsAg陽性率。結(jié)果80例新生兒中,HBsAg陽性6例,陽性率為7.5%;HBs Ab陽性45例,陽性率56.3%。不同喂養(yǎng)方式、分娩方式及產(chǎn)婦孕前是否按程序接種HBIG新生兒的HBsAg陽性率比較,差異無統(tǒng)計學意義(P0.05);產(chǎn)婦為"大三陽"新生兒的HBsAg陽性率高于產(chǎn)婦為"小三陽"的新生兒(P0.05)。結(jié)論乙肝疫苗聯(lián)合HBIG能夠降低HBV的母嬰垂直傳播率,孕婦"大三陽"http://www.cnki.com.cn/"小三陽"情況是新生兒HBsAg陽性率的影響因素。
[Abstract]:Objective to investigate the clinical efficacy and influencing factors of hepatitis B vaccine combined with hepatitis B immunoglobulin in blocking vertical transmission of hepatitis B from mother to child. Methods 80 cases of HBsAg positive pregnant women and their newborns were selected from June 2012 to June 2014 in Jiangshan people's Hospital. Hepatitis B immunoglobulin (HBIG),) was injected 24 hours after birth and inoculated according to "0-1-6" procedure. The HBsAg and HBs abs of newborns were detected, and the different feeding methods, delivery methods, the situation of "big three positive" http://www.cnki.com.cn/ "small three positive women" and the positive rate of HBsAg of pregnant women who were inoculated with HBIG before pregnancy were compared. Results among the 80 newborns, 6 cases were HBsAg positive, the positive rate was 7.5% and 45 cases were positive for HBs Ab, the positive rate was 56.3%. There was no significant difference in HBsAg positive rate of newborn infants with HBIG inoculated by procedure before delivery and delivery mode (P0.05), and the positive rate of HBsAg in newborns with "big three positive" pregnant women was higher than that of newborns with "small three positive" pregnant women (P0.05). Conclusion Hepatitis B vaccine combined with HBIG can reduce the vertical transmission rate of HBV from mother to child. The positive rate of HBsAg in neonates is influenced by the condition of "Big three positive" and "small three positive" of http://www.cnki.com.cn/ in pregnant women.
