【摘要】：目的:探討轉(zhuǎn)化生長(zhǎng)因子-β1(transforming growth factor-β1,TGF-β1)在宮腔粘連和子宮縱隔患者宮腔鏡術(shù)后子宮內(nèi)膜中的不同表達(dá)與預(yù)后之間的關(guān)系。方法:選取2014年6月至2015年5月在西安交通大學(xué)第二附屬醫(yī)院行宮腔鏡手術(shù)的患者96例,其中宮腔粘連、子宮縱隔患者各48例,用免疫組化SP法檢測(cè)TGF-β1在患者術(shù)后3周子宮內(nèi)膜中的表達(dá)情況,術(shù)后3個(gè)月按NASR評(píng)分將宮腔粘連和子宮縱隔患者分為預(yù)后良好組、預(yù)后一般組和預(yù)后不良組,用方差分析及卡方檢驗(yàn)分析各組數(shù)據(jù)之間的差異。結(jié)果:宮腔鏡術(shù)后3個(gè)月,子宮縱隔手術(shù)預(yù)后優(yōu)于宮腔粘連,差異有統(tǒng)計(jì)學(xué)意義(χ~2=13.714,P=0.000);免疫組化染色結(jié)果顯示,在宮腔粘連患者中,TGF-β1在預(yù)后不良組中的表達(dá)水平(n=16,2.94±0.70)明顯高于預(yù)后一般組(n=20,1.65±0.76)和預(yù)后良好組(n=12,1.08±0.79),差異有統(tǒng)計(jì)學(xué)意義(P=0.000,P=0.000),在預(yù)后一般組中的表達(dá)高于預(yù)后良好組,差異有統(tǒng)計(jì)學(xué)意義(P=0.045);在子宮縱隔患者中,TGF-β1在預(yù)后不良組中的表達(dá)水平(n=6,2.58±0.97)明顯高于預(yù)后一般組(n=12,1.25±0.54)和預(yù)后良好組(n=30,0.78±0.55),差異有統(tǒng)計(jì)學(xué)意義(P=0.000,P=0.000),在預(yù)后一般組中的表達(dá)高于預(yù)后良好組,差異有統(tǒng)計(jì)學(xué)意義(P=0.030)。結(jié)論:宮腔粘連和子宮縱隔宮腔鏡手術(shù)預(yù)后差異可能與TGF-β1在子宮內(nèi)膜中的異常表達(dá)有關(guān),其預(yù)后不良可能與TGF-β1在子宮內(nèi)膜中的過度表達(dá)有關(guān)。
[Abstract]:Objective: to investigate the relationship between the expression of transforming growth factor 尾 1 (transforming growth factor- 尾 1 (TGF- 尾 1) in endometrium and prognosis after hysteroscopy in patients with intrauterine adhesion and mediastinum. Methods: from June 2014 to May 2015, 96 patients underwent hysteroscopic surgery in the second affiliated Hospital of Xi'an Jiaotong University, including 48 cases of uterine cavity adhesion and 48 cases of uterine mediastinum. Immunohistochemical SP method was used to detect the expression of TGF- 尾 1 in endometrium 3 weeks after operation. According to NASR score, the patients with intrauterine adhesion and mediastinum were divided into three groups: good prognosis group, general prognosis group and poor prognosis group. Variance analysis and chi-square test were used to analyze the differences between groups. Results: three months after hysteroscopy, the prognosis of uterine mediastinal surgery was better than that of intrauterine adhesion (蠂 ~ (2 +) 13.714 ~ (0.000). The expression of TGF- 尾 1 in patients with intrauterine adhesion was significantly higher than that in patients with normal prognosis (n = 201.65 鹵0.76) and good prognosis (n = 120.08 鹵0.79). The expression of TGF- 尾 1 in poor prognosis group was higher than that in good prognosis group (P = 0.000, P 0.000), and the expression of TGF- 尾 1 in normal group was higher than that in good prognosis group (P < 0.05), and the expression of TGF- 尾 1 in patients with intrauterine adhesion was significantly higher than that in normal group with good prognosis (P < 0.05). The expression of TGF- 尾 1 in the patients with mediastinal uterine septum was significantly higher than that in the normal group (121.25 鹵0.54) and the group with good prognosis (0.78 鹵0.55), and the expression of TGF- 尾 1 in the group with poor prognosis was higher than that in the group with good prognosis (P 0.000 P 0.000), and the expression of TGF- 尾 1 in the normal group was higher than that in the group with good prognosis. The difference was statistically significant (P0. 030). Conclusion: the difference of prognosis between intrauterine adhesion and hysteroscopic surgery may be related to the abnormal expression of TGF- 尾 1 in endometrium, and the poor prognosis may be related to the overexpression of TGF- 尾 1 in endometrium.
