【摘要】：目的探討胎次與孕前BMI對(duì)妊娠期高血壓疾病的影響。方法以2011年2月—2012年12月在安徽省合肥市第一人民醫(yī)院進(jìn)行孕產(chǎn)期檢查并分娩的1 633名孕婦為研究對(duì)象,采用2檢驗(yàn)和logistic回歸模型分析胎次和孕前BMI與妊娠期高血壓疾病的關(guān)系。結(jié)果孕婦孕前BMI均數(shù)為(20.20±2.418)kg/m2,消瘦367人(22.5%),正常1 153人(70.6%),超重或肥胖113人(6.9%);第一胎936人(57.3%),第二胎及以上697人(42.7%);單因素logistic回歸模型結(jié)果顯示,孕前超重或肥胖的孕婦第一胎、二胎及以上發(fā)生妊娠期高血壓疾病的風(fēng)險(xiǎn)均升高(OR=2.722,95%CI=1.074~6.900;OR=4.356,95%CI=1.865~10.171);調(diào)整混雜因素孕婦實(shí)足年齡、文化程度、工作性質(zhì)后的多因素logistic回歸模型結(jié)果顯示,胎次是孕前超重或肥胖孕婦患妊娠期高血壓疾病的危險(xiǎn)因素(第2胎及以上:OR=3.535,95%CI=1.472~8.491,P=0.005)。結(jié)論孕前超重及肥胖婦女妊娠胎次越多發(fā)生妊娠期高血壓疾病的風(fēng)險(xiǎn)越高。
[Abstract]:Objective to investigate the effect of gestational BMI on hypertensive disorder complicating pregnancy. Methods from February 2011 to December 2012, 1 633 pregnant women who were examined and delivered in the first people's Hospital of Hefei, Anhui Province, were studied. 2 test and logistic regression model were used to analyze the relationship between gestational hypertension and BMI before pregnancy. Results the mean BMI before pregnancy was (20.20 鹵2.418) kg / m2, the mean value of BMI was (20.20 鹵2.418) kg / m ~ (2), the mean value of BMI was (20.20 鹵2.418) kg 路m ~ (2), the mean value of BMI was (73.5%), the normal rate was 1 153 (70.6%), the first pregnancy was overweight or obese (6.9%), the first pregnancy was 936 (57.3%), the second was 697 (42.7%), the single factor logistic regression model showed that the first child was overweight or obese before pregnancy. The risk of hypertensive disorder complicating pregnancy was increased in the second trimester and above (OR 2.722 ~ 95 CI 1.074 + 6.900 CI 4.356 + 95 CI 1.865 + 10.171), and the results of multivariate logistic regression model after adjusting for the pregnant women's actual age, education level and nature of work showed that, Birth order was the risk factor of hypertensive disorder complicating pregnancy in overweight or obese pregnant women before pregnancy. Conclusion the risk of hypertensive disorder complicating pregnancy is higher in overweight and obese women before pregnancy.
