多囊卵巢綜合征患者卵泡發(fā)育不良與SCF、BMP-15以及GDF-9表達(dá)水平的相關(guān)性研究

【摘要】:目的:探討多囊卵巢綜合征(PCOS)患者卵泡中干細(xì)胞因子(SCF)、骨形態(tài)發(fā)生蛋白-15(BMP-15)和生長(zhǎng)分化因子-9(GDF-9)與卵泡發(fā)育異常的相關(guān)性。方法:分別收集經(jīng)體外受精(IVF)治療的30例PCOS患者(PCOS組)和40例對(duì)照患者(對(duì)照組)的卵泡液和顆粒細(xì)胞。分析兩組患者一般指標(biāo)情況(包括獲卵數(shù),年齡,血清卵泡刺激素,黃體生成素,雌二醇,泌乳素,睪酮以及Gn量)以及卵母細(xì)胞MII率,受精率和優(yōu)質(zhì)胚胎率并進(jìn)行比較。采用酶聯(lián)免疫吸附實(shí)驗(yàn)檢測(cè)卵泡液中SCF,BMP-15和GDF-9的表達(dá)水平,應(yīng)用免疫熒光染色法測(cè)定顆粒細(xì)胞中SCF的表達(dá)水平,并進(jìn)一步通過(guò)蛋白質(zhì)印跡法比較兩組SCF蛋白表達(dá)水平。結(jié)果:(1)與對(duì)照組(n=40)相比,PCOS組(n=30)血清LH,T水平顯著增高(P0.05)。其余指標(biāo)均在統(tǒng)計(jì)學(xué)上無(wú)意義(P0.05);(2)對(duì)照組和PCOS組卵母細(xì)胞MII率分別為84.67±13.95%和71.10±12.53%,受精率分別為74.37±9.33%和63.26±6.07%,優(yōu)質(zhì)胚胎率分別為35.11±3.04%和33.85±2.59%。由此可知,PCOS組卵母細(xì)胞MII率和受精率較對(duì)照組明顯降低(P0.05),然而,兩組之間優(yōu)質(zhì)胚胎率無(wú)明顯差異(P0.05);(3)對(duì)照組和PCOS組卵泡液中SCF的平均濃度分別為23.05±2.86ng/L和21.17±3.11ng/L,PCOS患者卵泡液中SCF的濃度明顯低于正常對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。免疫熒光檢測(cè)發(fā)現(xiàn),與對(duì)照組相比,PCOS患者顆粒細(xì)胞中SCF的表達(dá)水平顯著下降,隨后的蛋白質(zhì)印跡法實(shí)驗(yàn)進(jìn)一步驗(yàn)證這一結(jié)果;(4)卵泡液中對(duì)照組和PCOS組BMP-15平均濃度分別為132.53±32.51ng/L和115.50±27.61ng/L。而GDF-9平均濃度分別為138.28±27.81ng/L和121.97±19.66ng/L,可以看出卵泡液中BMP-15和GDF-9的濃度均明顯低于正常對(duì)照組(P0.05)。結(jié)論:(1)PCOS患者卵泡發(fā)育不良,卵子質(zhì)量差,但優(yōu)質(zhì)胚胎率與對(duì)照組相比無(wú)明顯差異,提示兩組胚胎的發(fā)育潛能可能無(wú)差異;(2)PCOS組卵泡液和顆粒細(xì)胞中SCF的表達(dá)均明顯降低,提示SCF可能與PCOS卵泡發(fā)育異常有關(guān);(3)PCOS患者BMP-15和GDF-9表達(dá)水平下調(diào),提示該因子亦可能參與調(diào)控PCOS患者卵泡異常發(fā)育的過(guò)程。
[Abstract]:Objective: to investigate the relationship between (SCF), bone morphogenetic protein-15 (BMP-15) and growth differentiation factor-9 (GDF-9) and follicular dysplasia in patients with polycystic ovary syndrome (PCOS). Methods: follicular fluid and granulosa cells were collected from 30 PCOS patients (PCOS group) and 40 control patients (control group) treated with in vitro fertilization (IVF). The general indexes (including egg count, age, serum follicle stimulating hormone, luteinizing hormone, estradiol, prolactin, testosterone and Gn) and oocyte MII rate, fertilization rate and high quality embryo rate were analyzed and compared between the two groups. Enzyme linked immunosorbent assay (Elisa) was used to detect the expression of SCF- BMP-15 and GDF-9 in follicular fluid. The expression of SCF in granulosa cells was detected by immunofluorescence staining. Furthermore, the expression levels of SCF protein in the two groups were compared by Western blotting. Results: (1) compared with the control group (nm40), the serum LHN T level in the PCOS group was significantly higher than that in the control group (P0.05). The MII rates of oocytes in the control group and PCOS group were 84.67 鹵13.95% and 71.10 鹵12.53, respectively. The fertilization rates were 74.37 鹵9.33% and 63.26 鹵6.07%, respectively. The high quality embryo rates were 35.11 鹵3.04% and 33.85 鹵2.59%, respectively. Therefore, the oocyte MII rate and fertilization rate in PCOS group were significantly lower than those in the control group (P0.05), however, There was no significant difference in the rate of high quality embryos between the two groups (P0.05); (3). The average concentration of SCF in follicular fluid of control group and PCOS group was 23.05 鹵2.86ng/L and 21.17 鹵3.11ng / L respectively, and the concentration of SCF in follicular fluid of PCOS group was significantly lower than that of normal control group (P0.05). Immunofluorescence assay showed that the expression of SCF in granulosa cells of PCOS patients was significantly decreased compared with the control group, which was further verified by Western blotting assay. (4) the average BMP-15 concentration in follicular fluid was 132.53 鹵32.51ng/L and 115.50 鹵27.61 ng / L in the control group and PCOS group, respectively. The average concentration of GDF-9 was 138.28 鹵27.81ng/L and 121.97 鹵19.66ng / L, respectively, which showed that the concentrations of BMP-15 and GDF-9 in follicular fluid were significantly lower than those in normal control group (P0.05). Conclusion: (1) follicular dysplasia and poor egg quality in PCOS patients, but there is no significant difference in the rate of high quality embryos between the two groups, suggesting that there may be no difference in the developmental potential between the two groups, and (2) the expression of SCF in follicular fluid and granulosa cells in the PCOS group is significantly lower than that in the control group. The results suggest that SCF may be related to the abnormal development of PCOS follicles, (3) the expression of BMP-15 and GDF-9 in PCOS patients is down-regulated, which suggests that this factor may also be involved in the regulation of abnormal follicle development in PCOS patients.
