【摘要】：目的:探討HMG誘導(dǎo)排卵療效相關(guān)證型分布規(guī)律及尺膚診的臨床診斷價(jià)值,為各種臨床表癥的排卵障礙性不孕癥患者是否應(yīng)采取HMG促排治療及促排失敗后如何選擇最佳的中醫(yī)后續(xù)治療方案提供證候?qū)W依據(jù)和辨證思路。方法:采集接受尿促性素(HMG)誘導(dǎo)排卵治療成功病例(70例)和失敗病例(180例)的癥狀、舌象、脈象等四診信息,進(jìn)行Logistic回歸分析、因子分析、聚類分析等統(tǒng)計(jì)學(xué)處理。結(jié)果:1.兩組間諸多四診信息差異有統(tǒng)計(jì)學(xué)意義。多元回歸分析結(jié)果顯示:口渴喜冷、腰酸腿軟、脈數(shù)、脈洪與療效負(fù)相關(guān),脈細(xì)、苔薄白與療效正相關(guān)。2.誘導(dǎo)排卵失敗患者的中醫(yī)證型分布為熱盛津傷型占40.00%、肝郁化熱占32.78%、腎陰虛內(nèi)熱型占27.22%,成功組患者的中醫(yī)證型分布為陽虛兼肝郁占91.43%、陰津虧虛占1.43%、陰虛化熱占2.86%、肝郁兼熱盛占4.29%。結(jié)論:1.中醫(yī)四診信息與誘導(dǎo)排卵療效具有明顯的相關(guān)性,對誘導(dǎo)排卵療效具有預(yù)測價(jià)值。2.兩組均有各自的中醫(yī)證型分布規(guī)律,中醫(yī)辨證對誘導(dǎo)排卵療效具預(yù)測價(jià)值。
[Abstract]:Objective: to investigate the distribution of syndromes related to the efficacy of HMG induced ovulation and the clinical diagnostic value of ulnar skin diagnosis. To provide syndromes basis and syndrome differentiation for patients with ovulatory disorder infertility of various clinical epiderms whether to take HMG to promote ovulation treatment and how to choose the best follow-up treatment scheme after failure. Methods: four diagnoses of symptoms, tongue and pulse were collected in 70 cases of successful ovulation induced by urotropin (HMG) and 180 cases of failure. Logistic regression analysis, factor analysis and cluster analysis were performed. The result is 1: 1. There were significant differences in four-diagnosis information between the two groups. The results of multiple regression analysis showed that: thirst and cold, weak waist and leg, pulse number, pulse flood were negatively correlated with curative effect, pulse size, thin fur and white fur were positively correlated with curative effect. 2. The distribution of TCM syndromes of failed ovulation patients was heat Shengjin injury (40.00g), liver depression and heat (32.78m), kidney yin deficiency (27.22g). In successful group, the TCM syndromes were Yang deficiency and liver depression (91.43%), Yin deficiency (1.43%), Yin deficiency heat (2.86%), liver depression and heat (4.2929%). Conclusion 1. The information of four diagnoses of TCM has a significant correlation with the effect of ovulation induction, and has predictive value. 2. 2. The two groups have their own distribution of TCM syndromes, TCM syndrome differentiation can predict the efficacy of ovulation induction.
