ABO血型與卵巢儲(chǔ)備功能相關(guān)性的meta分析

【摘要】:背景:卵巢儲(chǔ)備功能是指卵巢中卵子的質(zhì)量以及可以用于生殖的卵母細(xì)胞的數(shù)量。近年來(lái),社會(huì)環(huán)境的變化及女性自身?xiàng)l件的改變導(dǎo)致女性不孕趨勢(shì)逐年增長(zhǎng)。據(jù)統(tǒng)計(jì)大約10%的不孕女性卵巢儲(chǔ)備功能下降。卵巢儲(chǔ)備功能的預(yù)測(cè)指標(biāo)研究較多的包括卵泡刺激素及抗苗勒管激素。ABO血型系統(tǒng)是一種非常穩(wěn)定的遺傳標(biāo)記,研究表明ABO血型與心血管疾病、胃癌、胰腺癌等疾病有關(guān)聯(lián)。近些年有研究探討ABO血型與卵巢儲(chǔ)備功能之間的關(guān)系,但并未達(dá)成共識(shí)。血型是否與卵巢儲(chǔ)備功能有關(guān),哪種抗原是保護(hù)或危險(xiǎn)因素,尚存在爭(zhēng)議。目的:本文首次進(jìn)行Meta分析以闡明ABO血型與卵巢儲(chǔ)備功能之間的相關(guān)性。方法:檢索中英文電子數(shù)據(jù)庫(kù),以“ABO blood group”、“ovarian reserve”、“血型”、“卵巢儲(chǔ)備”為主題詞加自由詞進(jìn)行檢索,并手工搜索相關(guān)研究的參考文獻(xiàn),沒(méi)有語(yǔ)言和國(guó)家的限制。主要分析研究結(jié)果是卵巢儲(chǔ)備功能下降的發(fā)生率,研究的效應(yīng)量懫用比值比(OR)及95%可信區(qū)間(CI)評(píng)價(jià)。統(tǒng)計(jì)分析使用Review Manager 5.3版本,采用Q檢驗(yàn)和I2分析研究間的異質(zhì)性。當(dāng)P≥0.1和I2≤50%被認(rèn)為是沒(méi)有異質(zhì)性的研究;P0.1和I250%的研究被認(rèn)為有異質(zhì)性。由于隨機(jī)效應(yīng)模型相對(duì)較保守,使結(jié)果更傾向于安全,故采用隨機(jī)效應(yīng)模型。通過(guò)敏感性分析評(píng)估每一項(xiàng)研究對(duì)整體分析的影響來(lái)尋找異質(zhì)性來(lái)源。結(jié)果:最終Meta分析納入七篇橫斷面研究,來(lái)自四個(gè)國(guó)家涉及55113人,采用卵泡刺激素及抗苗勒管激素兩種檢測(cè)指標(biāo)判定卵巢儲(chǔ)備功能。以卵泡刺激素測(cè)定卵巢儲(chǔ)備功能的五篇研究進(jìn)行分析,結(jié)果顯示A型血OR=1.03(95%CI:0.96~1.11),I2=17%;B型血OR=0.98(95%CI:0.75~1.29),I2=88%;AB型血OR=0.96(95%CI:0.71~1.30),I2=75%;O型血OR=1.03(95%CI:0.74~1.43),I2=94%。以抗苗勒管激素測(cè)定卵巢儲(chǔ)備功能的兩篇研究進(jìn)行分析,結(jié)果顯示A型血OR=0.89(95%CI:0.76~1.03),I2=0%;B型血OR=1.02(95%CI:0.80~1.30),I2=24%;AB型血OR=1.14(95%CI:0.80~1.64),I2=0%;O型血OR=1.07(95%CI:0.86~1.34),I2=46%。結(jié)論:以現(xiàn)有的研究未發(fā)現(xiàn)ABO血型與卵巢儲(chǔ)備功能之間的關(guān)聯(lián)。然而,需要更多、更嚴(yán)謹(jǐn)、高質(zhì)量、大樣本和多指標(biāo)的研究進(jìn)行進(jìn)一步的驗(yàn)證。
[Abstract]:Background: ovarian reserve function refers to the quality of eggs in the ovary and the number of oocytes that can be used to reproduce. In recent years, the change of social environment and women's own conditions lead to the increasing trend of female infertility. According to statistics, about 10% of infertile women ovarian reserve function decline. The predictors of ovarian reserve function include follicle stimulating hormone (FSH) and anti-Mullerian hormone. ABO blood group system is a very stable genetic marker. It is shown that ABO blood group is associated with cardiovascular disease, gastric cancer, pancreatic cancer and other diseases. Recent studies have explored the relationship between ABO blood group and ovarian reserve function, but there is no consensus. Whether blood type is related to ovarian reserve function, which antigens are protective or risk factors remains controversial. Objective: to investigate the correlation between ABO blood group and ovarian reserve function by meta-analysis for the first time. Methods: Chinese and English electronic databases were searched with "ABO blood group", "ovarian reserve", "blood group" and "ovary reserve" as subject words plus free words.
