【摘要】：目的 :探討磷脂酰肌醇蛋白聚糖3(glypican-3,GPC3)在卵巢透明細(xì)胞腺癌中的表達(dá)及臨床意義。方法 :采用免疫組織化學(xué)方法分別檢測83例卵巢癌組織(透明細(xì)胞腺癌45例、漿液性癌20例、黏液性癌18例)、20例漿液性囊腺瘤及20例正常卵巢組織中GPC3蛋白的表達(dá)情況,實(shí)時(shí)熒光定量PCR法檢測38例卵巢癌組織(透明細(xì)胞腺癌20例、漿液性癌10例、黏液性癌8例)、5例漿液性囊腺瘤和5例正常卵巢組織中m RNA的表達(dá)情況,并分析GPC3與卵巢透明細(xì)胞腺癌臨床病理學(xué)參數(shù)及預(yù)后的相關(guān)性。結(jié)果:1卵巢透明細(xì)胞腺癌中GPC3蛋白陽性表達(dá)率和m RNA表達(dá)量(82.2%;1.326 1±0.493 6)均明顯高于漿液性癌(20%;0.426 5±0.029 4)、黏液性癌(5%;0.265 2±0.103 9)、漿液性囊腺瘤(0%;0.141 3±0.011 3)、正常卵巢組織(0%;0.291 4±0.048 1),差異有統(tǒng)計(jì)學(xué)意義(P均0.01);2GPC3蛋白陽性表達(dá)率及GPC3 m RNA表達(dá)量與卵巢透明細(xì)胞腺癌的臨床分期有相關(guān)性,Ⅲ~Ⅳ期患者中的表達(dá)(100%;1.808 1±0.265 7)顯著高于Ⅰ~Ⅱ期患者(71.4%;1.045 8±0.403 6)(P值0.01);3卵巢透明細(xì)胞腺癌鉑類藥物耐藥患者GPC3蛋白陽性表達(dá)率及m RNA表達(dá)量(100%;1.808 1±0.265 7)均高于鉑類藥物敏感型患者(70.3%;0.991 8±0.330 3)(P0.05);4GPC3蛋白陽性表達(dá)率與卵巢透明細(xì)胞腺癌患者的預(yù)后相關(guān),預(yù)后差的患者中陽性率及表達(dá)量高,差異有統(tǒng)計(jì)學(xué)意義(P0.05),Kaplan-Meier單因素生存分析亦顯示GPC3蛋白為影響預(yù)后的因素(P=0.048);5卵巢透明細(xì)胞腺癌中GPC3蛋白陽性表達(dá)率和m RNA表達(dá)量與患者的淋巴結(jié)轉(zhuǎn)移和遠(yuǎn)處器官轉(zhuǎn)移無相關(guān)性(P0.05)。結(jié)論 :GPC3在病理鑒別診斷中有重要意義,GPC3與卵巢透明細(xì)胞腺癌患者的臨床分期和鉑類耐藥相關(guān),提示其可能在卵巢透明細(xì)胞腺癌的發(fā)生發(fā)展起重要作用,有望成為卵巢透明細(xì)胞腺癌的預(yù)后指標(biāo)和潛在治療靶標(biāo)。
[Abstract]:Objective: to investigate the expression and clinical significance of phosphatidylinositol proteoglycan 3 (glypican-3) in ovarian clear cell adenocarcinoma. Methods: the expression of GPC3 protein in 83 cases of ovarian carcinoma (45 cases of clear cell adenocarcinoma, 20 cases of serous carcinoma, 18 cases of mucinous carcinoma) and 20 cases of serous cystadenoma and 20 cases of normal ovary were detected by immunohistochemical method. The expression of mRNA in 38 cases of ovarian carcinoma (20 cases of clear cell adenocarcinoma, 10 cases of serous carcinoma, 8 cases of mucinous carcinoma) and 5 cases of serous cystadenoma and 5 cases of normal ovary were detected by real-time fluorescence quantitative PCR. The relationship between GPC3 and clinicopathological parameters and prognosis of ovarian clear cell adenocarcinoma was analyzed. Results the positive expression rate and mRNA expression of GPC3 protein in ovarian clear cell adenocarcinoma were significantly higher than those in serous carcinoma (0.493 鹵0.029 4), mucinous carcinoma (0.265 2 鹵0.103 9), serous cystadenoma (0.0.141 3 鹵0.011 3) and normal ovary (0.291 4 鹵0.048 1). The positive expression rate of GPC3 protein and the expression of GPC3 mRNA were correlated with the clinical stage of ovarian clear cell adenocarcinoma.
