【摘要】：目的研究孕中期妊娠期糖尿病孕婦血清鐵、鋅和鐵蛋白的變化情況,探討其與妊娠期糖尿病發(fā)病的關(guān)系。方法選擇2014年1月-2015年12月該院24~28周妊娠期糖尿病孕婦200例作為妊娠期糖尿病組,血糖正常孕婦200例作為對照組。觀察兩組孕婦血清鐵蛋白、血清鐵、血清鋅水平及其與糖代謝指標(biāo)的相關(guān)性,比較兩組妊娠期鐵缺乏、缺鐵性貧血及胎兒發(fā)育情況。結(jié)果妊娠期糖尿病組孕婦空腹血糖(5.25±3.21)mmol/L、1 h血糖(10.56±1.73)mmol/L、2 h血糖(8.51±0.87)mmol/L、空腹胰島素(13.12±2.24)mmol/L、糖化血紅蛋白(6.97%)、胰島素抵抗指數(shù)(2.98)均高于對照組(4.29±1.05)mmol/L、(8.72±1.24)mmol/L、(6.98±0.82)mmol/L、(6.46±1.87)mmol/L、(4.26%)、(1.19)(P0.05),妊娠期糖尿病組胰島β細(xì)胞分泌功能(147.87)低于對照組(164.50)(P0.05)。妊娠期糖尿病組患者血清鐵(27.34±7.12)μmol/L和鐵蛋白(59.16±5.78)ng/ml水平高于對照組(20.98±6.01)μmol/L和(40.43±4.87)ng/ml(P0.05),妊娠期糖尿病組患者血清鋅(10.59±2.23)μmol/L水平低于對照組(14.02±2.43)μmol/L(P0.05)。血清鐵蛋白和血清鐵與空腹血糖、空腹胰島素、糖化血紅蛋白、胰島素抵抗指數(shù)均呈正相關(guān)(P0.05)。血清鋅和空腹血糖、空腹胰島素、糖化血紅蛋白、胰島素抵抗指數(shù)均呈負(fù)相關(guān)(P0.05)。兩組妊娠期鐵缺乏、缺鐵性貧血的發(fā)生率差異無統(tǒng)計學(xué)意義(P0.05),兩組胎兒雙頂徑、股骨長、頭圍、腹圍差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論孕中期妊娠期糖尿病孕婦血清鐵和鐵蛋白水平升高,血清鋅水平下降,血清鐵、鋅、鐵蛋白水平與妊娠期糖尿病孕婦的糖代謝指標(biāo)有相關(guān)性,和妊娠期糖尿病的發(fā)病有關(guān)。
[Abstract]:Objective to study the changes of serum iron, zinc and ferritin in pregnant women with gestational diabetes mellitus (M2DM), and to explore the relationship between them and the pathogenesis of gestational diabetes mellitus (GDM). Methods from January 2014 to December 2015, 200 pregnant women with gestational diabetes mellitus were selected as gestational diabetes mellitus group and 200 normal pregnant women as control group.
