【摘要】：目的探討α-L-巖藻糖苷酶(AFU)在妊娠糖尿病(GDM)中的檢測(cè)價(jià)值以及AFU與血總膽汁酸(TBA)的相關(guān)性。方法 360例孕婦于孕12周前測(cè)量身高、體質(zhì)量、腰圍及血壓;孕24~28周行口服葡萄糖耐量試驗(yàn)(OGTT),檢測(cè)0、60及120 min血糖及胰島素、血常規(guī)及生化指標(biāo),確認(rèn)GDM患者,按年齡及體質(zhì)量指數(shù)匹配對(duì)照組。對(duì)二組進(jìn)行比較并作統(tǒng)計(jì)分析。結(jié)果 GDM組AFU水平高于糖耐量正常(NGT)組,對(duì)GDM的診斷有參考價(jià)值且與餐后1 h血糖相關(guān);GDM組直接膽紅素低于NGT組;校正肝功能及糖化血紅蛋白后,AFU與TBA相關(guān)。結(jié)論孕期AFU水平對(duì)GDM的診斷有參考價(jià)值,主要與餐后1 h血糖水平相關(guān);AFU水平升高同時(shí)伴有直接膽紅素下降、TBA及堿性磷酸酶水平增加,提示GDM患者膽道功能呈活躍狀態(tài)。
[Abstract]:Objective to investigate the value of 偽-L-fucosidase (AFU) in the detection of gestational diabetes mellitus (GDM) and the correlation between AFU and total bile acid (TBA). Methods the body height, body mass, waist circumference and blood pressure were measured in 360 pregnant women before 12 weeks of gestation, and oral glucose tolerance test (OGTT) was performed at the 28th week of gestation. Blood glucose and insulin, blood routine and biochemical indexes were detected at 60 and 120 min. The control group was matched according to age and body mass index. The two groups were compared and statistically analyzed. Results the level of AFU in GDM group was higher than that in NGT group, which had reference value in diagnosis of GDM and was lower than that in NGT group after 1 h postprandial blood glucose, and there was a correlation between AFU and TBA after correcting liver function and glycosylated hemoglobin. Conclusion the level of AFU in pregnancy is valuable for the diagnosis of GDM, which is mainly related to the elevation of AFU level at 1 h postprandial and accompanied by the increase of direct bilirubin decreasing TBA and alkaline phosphatase, indicating that the biliary tract function of GDM patients is active.
