ING4、PHB與Ki-67在宮頸病變組織中的表達(dá)與HR-HPV感染的相關(guān)性研究

【摘要】:目的:探討宮頸病變組織中的ING4、PHB、Ki-67的表達(dá)及其與HR-HPV感染的相關(guān)性。方法:選取2010年1月-2015年10月在我院經(jīng)病理診斷為宮頸病變的132例的患者宮頸組織,分為慢性宮頸炎39例,宮頸上皮內(nèi)瘤變(cervical intraepithelial neoplasia,CIN)1級23例,CIN2-3級28例,宮頸鱗癌42例。采用免疫組織化學(xué)檢測技術(shù)檢測不同宮頸病變組織中生長抑制因子4(Inhibitor of growth family member 4,ING 4)、抗增殖蛋白(Prohibitin,PHB)、Ki-67的表達(dá)情況,采用雜交捕獲二代(Hrbird Capture2,HC2)技術(shù)檢測患者宮頸分泌物中高危型人乳頭瘤病毒(high-risk human papillomavirus,HR-HPV)的感染情況,同時(shí)檢測HPV-DNA的裁量。結(jié)果:1.ING4在正常宮頸上皮組織的細(xì)胞核中陽性表達(dá)。其在不同宮頸病變組織中表達(dá)不同,在慢性宮頸炎、CIN 1級、CIN 2-3級、宮頸鱗癌組中ING4表達(dá)的中強(qiáng)陽性表達(dá)率分別為:74.35%、60.87%、7.14%、54.76%,總體差異有統(tǒng)計(jì)學(xué)意義(Hc=34.805,P0.01),進(jìn)一步兩兩比較顯示,慢性宮頸炎組與CIN 2-3級組、慢性宮頸炎組與宮頸鱗癌組、CIN 1級組與宮頸鱗癌組、CIN 2-3級與宮頸鱗癌組的差異均有意義(P0.01)在慢性宮頸炎與宮頸癌前病變的三組中,ING4的表達(dá)與病變程度呈負(fù)相關(guān)(r=-0.228,P0.05),而在宮頸癌組織中,ING4表達(dá)較癌前病變組織異常升高。2.PHB在宮頸上皮組織中的細(xì)胞漿中陽性表達(dá)。在慢性宮頸炎、CIN 1級、CIN 2-3級、宮頸鱗癌組中PHB表達(dá)的強(qiáng)陽性率(+++)分別為35.9%、47.8%、57.1%和71.4%,總體差異有統(tǒng)計(jì)學(xué)意義(Hc=11.552,P0.01),且PHB的陽性表達(dá)程度與宮頸組織病變程度呈正相關(guān)(r=0.291,P0.01)。3.Ki-67的陽性表達(dá)主要定位于宮頸上皮組織的細(xì)胞核中,其在慢性宮頸炎、CIN 1級、CIN 2-3級、宮頸鱗癌組中Ki-67表達(dá)的陽性率分別為7.7%、73.9%、100%和100%,其中強(qiáng)陽性率(≥++)分別為2.6%、13%、75%和97.6%,總體差異有統(tǒng)計(jì)學(xué)意義(Hc=99.177,P0.01),Ki-67與宮頸病變的嚴(yán)重程度呈正相關(guān)(r=0.860,P0.01)。4.HR-HPV DNA的裁量與宮頸病變程度呈正相關(guān)(r=0.445,P0.01)。5.在宮頸組織中PHB、ING4與HR-HPV不相關(guān)。Ki-67與HR-HPV呈正相關(guān)(r=0.353,P0.01)。ING4與Ki-67的表達(dá)呈負(fù)相關(guān)(r=-0.256,P0.01),但PHB與Ki-67的表達(dá)之間呈正相關(guān)(r=0.369,P0.01),ING4與PHB的表達(dá)之間不相關(guān)。6.在宮頸癌組織中,ING4的表達(dá)與宮頸鱗癌組織的分化程度呈負(fù)相關(guān)(P0.05),組織分化程度越低,ING4在宮頸上皮組織中的表達(dá)越少,ING4的表達(dá)與宮頸癌患者的年齡、FIGO分期、腫瘤大小、浸潤程度、淋巴結(jié)轉(zhuǎn)移均無顯著相關(guān)。PHB在宮頸鱗癌組的表達(dá)與年齡、分化程度、臨床分期、腫瘤大小、浸潤程度、淋巴結(jié)轉(zhuǎn)移亦均無顯著相關(guān)性。結(jié)論:1.PHB、ING4、Ki-67在宮頸上皮組織中的表達(dá)及HR-HPV的感染在宮頸癌的發(fā)生發(fā)展中有重要作用。2.ING4、PHB可能通過Ki-67與HR-HPV互相協(xié)同致癌或者抑癌。3.在宮頸癌中,ING4、PHB可能參與了宮頸癌的發(fā)生發(fā)展,有可能成為新的宮頸癌潛在的診斷標(biāo)記物及治療靶點(diǎn)。
[Abstract]:Objective: to investigate the expression of Ki-67 and its correlation with HR-HPV infection in cervical lesions. Methods: 132 cases of cervical lesions diagnosed pathologically in our hospital from January 2010 to October 2015 were divided into three groups: 39 cases of chronic cervicitis, 23 cases of cervical intraepithelial neoplasia (cervical intraepithelial plasia cin) grade 1, 28 cases of CIN2-3 grade, and 42 cases of cervical squamous cell carcinoma. Immunohistochemical technique was used to detect the expression of inhibitor of growth family member 4 and anti proliferating protein Ki-67 in different cervical lesions. Results 1. ING4 was expressed in the nuclei of normal cervical epithelium. The positive rate of ING4 expression in cervical squamous cell carcinoma group was 74.35%, 60.87% and 7.14%, respectively. However, the expression of ING4 in cervical carcinoma was significantly higher than that in precancerous lesions. 2. PHB was positively expressed in the cytoplasm of cervical epithelium. In chronic cervicitis, CIN1, CIN2-3, The strong positive rate of PHB expression in cervical squamous cell carcinoma group was 35.9% and 71.4%, respectively.
