【摘要】：目的觀察根治術(shù)后同步放化療與放療序貫化療對(duì)老年宮頸癌患者生存情況的影響。方法選擇行宮頸癌根治術(shù)后需放化療治療的老年早期宮頸癌患者98例,按照患者術(shù)后治療方式分為同步放化療組(同步組)50例、放療序貫化療組(序貫組)48例,同步組放療、化療同步進(jìn)行,而序貫組先行放療,放療結(jié)束后再給予輔助化療。比較兩組治療前后細(xì)胞免疫功能、毒副作用發(fā)生情況及隨訪生存情況。結(jié)果治療后兩組CD3~+、CD4~+、CD4~+/CD8~+水平較治療前均顯著降低(P0.05),而CD8~+、自然殺傷(NK)細(xì)胞比例較治療前顯著升高(P0.05),治療后兩組CD3~+、CD4~+、CD8~+、CD4~+/CD8~+及NK細(xì)胞比例均無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)。兩組毒副反應(yīng)主要為胃腸道反應(yīng)、骨髓抑制及放射性直腸炎,且均以Ⅰ~Ⅱ度毒副作用為主,且兩組均無(wú)統(tǒng)計(jì)學(xué)差異(P0.05);同步組3年局部無(wú)進(jìn)展生存率為78.00%,序貫組為68.00%,兩組差異顯著(P0.05);同步組3年總體生存率為88.00%,序貫組為70.83%,兩組差異顯著(P0.05)。結(jié)論對(duì)于老年早期高危宮頸癌患者,術(shù)后給予患者同步放化療可明顯提高局部無(wú)進(jìn)展生存率以及總體生存情況,且采取同步放化療與序貫放化療對(duì)患者免疫功能影響無(wú)差異,治療毒副作用雖略有增加,但積極對(duì)癥處理后患者耐受情況較好。
[Abstract]:Objective to observe the effect of concurrent radiotherapy and chemotherapy and radiotherapy sequential chemotherapy on survival of elderly patients with cervical cancer after radical operation. Methods 98 elderly patients with early cervical cancer who needed radiotherapy and chemotherapy after radical resection of cervical cancer were divided into two groups: simultaneous radiotherapy and chemotherapy group (n = 50), sequential chemotherapy group (n = 48) and radiotherapy group (n = 48). Chemotherapy was performed simultaneously, followed by radiotherapy in the sequential group, followed by adjuvant chemotherapy at the end of radiotherapy. The cellular immune function, side effects and survival were compared before and after treatment between the two groups. Results after treatment, the levels of CD3 ~ + CD4 ~ + CD4- / CD8- / CD8- were significantly lower than those before treatment (P0.05), while the percentage of CD8-, natural killer (NK) cells increased significantly (P0.05). After treatment, there was no significant difference between the two groups in the percentage of CD3 ~ + CD4- / CD4- / CD8- and NK cells (P0.05).
