【摘要】：目的總結(jié)探討雙球囊導(dǎo)管用于足月妊娠促宮頸成熟及引產(chǎn)的有效性、安全性和對產(chǎn)程的影響。方法選擇2015年1月至2016年12月期間在青島大學(xué)附屬醫(yī)院住院并有引產(chǎn)指征的140例足月妊娠孕婦為研究對象,隨機分為觀察組和對照組各70例,其中對照組采取陰道后穹窿放置地諾前列酮栓(普貝生)來促宮頸成熟,觀察組選擇在宮頸陰道內(nèi)置入雙球囊導(dǎo)管(COOK球囊),利用內(nèi)外側(cè)機械擠壓作用促進宮頸成熟,觀察比較兩組孕婦宮頸Bishop評分變化、引產(chǎn)療效、產(chǎn)程、分娩結(jié)局等情況。結(jié)果觀察組雙導(dǎo)管球囊取出后的宮頸Bishop評分(7.8±0.5)明顯高于對照組地諾前列酮栓取出后的宮頸Bishop評分(7.1±0.3)分,差異有統(tǒng)計學(xué)意義(P0.01);觀察組引產(chǎn)總有效率90.00%明顯高于對照組引產(chǎn)總有效率77.14%,臨產(chǎn)時間(10.5±2.5)h及總產(chǎn)程(8.1±0.5)h均明顯短于對照組,剖宮產(chǎn)率17.14%明顯低于對照組,差異均有統(tǒng)計學(xué)意義(P0.05)。結(jié)論雙球囊導(dǎo)管用于足月妊娠促宮頸成熟的效果良好,改善宮頸條件和縮短產(chǎn)程,且安全性較高,值得推廣使用。
[Abstract]:Objective to evaluate the efficacy, safety and influence of double balloon catheter in term pregnancy on cervical maturation and labor induction. Methods from January 2015 to December 2016, 140 pregnant women who were hospitalized in the affiliated Hospital of Qingdao University and who had the indication of induced labor were randomly divided into two groups: the observation group and the control group. In the control group, the posterior fornix of vagina was placed with fenoprostone suppository (Puberson) to promote cervical maturation, while in the observation group, two balloon catheters were placed in the cervix vagina to promote cervical maturation by mechanical extrusion of the inner and outer side. The changes of cervical Bishop score, the effect of induced labor, labor process and delivery outcome were observed and compared between the two groups. Results the cervical Bishop score (7.8 鹵0.5) after double catheter balloon removal in the observation group was significantly higher than that in the control group (7.1 鹵0.3). The total effective rate of induced labor in the observation group was 90.00% significantly higher than that in the control group (77.14%), the time of labor was 10.5 鹵2.5 hours and the total labor process was 8.1 鹵0.5 hours, and the cesarean section rate was 17.14% lower than that in the control group (P 0.05). Conclusion double balloon catheter is effective in promoting cervix maturation in term pregnancy. It can improve cervical condition and shorten labor process. It is safe and worth popularizing.
