骨代謝標(biāo)志物在不同妊娠次數(shù)中對(duì)胎兒長(zhǎng)骨發(fā)育的影響

【摘要】:目的通過(guò)測(cè)定初次妊娠及二次妊娠婦女骨代謝標(biāo)志物、骨密度值的水平了解孕婦的骨骼狀態(tài)及同期胎兒長(zhǎng)骨的發(fā)育情況,分析妊娠次數(shù)是否會(huì)對(duì)胎兒長(zhǎng)骨及孕婦自身骨密度產(chǎn)生影響。方法隨機(jī)選取2016年2月至2016年12月在寧夏醫(yī)科大學(xué)總醫(yī)院就診的妊娠婦女520例,根據(jù)排除標(biāo)準(zhǔn)排除16例,剩余504例作為研究對(duì)象。根據(jù)妊娠次數(shù)將妊娠婦女分為兩組,初次妊娠455例,二次妊娠49例。分別對(duì)兩組的骨代謝標(biāo)記物、胎兒長(zhǎng)骨數(shù)值進(jìn)行比較分析。其中產(chǎn)后1年出現(xiàn)腰酸背痛、腓腸肌痙攣的產(chǎn)婦共104例,初次妊娠91例,二次妊娠13例,比較兩組妊娠前、產(chǎn)后3個(gè)月、產(chǎn)后1年的骨密度值。結(jié)果1.二次妊娠組的年齡均大于初次妊娠組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05),而孕周在兩組間無(wú)顯著性差異(P0.05)。2.初次妊娠組、二次妊娠組的維生素D水平均小于正常值(≤30ng/ml),兩組維生素D水平相比較,無(wú)顯著性差異(P0.05)。3.初次妊娠組、二次妊娠組的血清鈣、磷、鎂水平相比較,無(wú)顯著性差異(P0.05)。4.初次妊娠組的CTX-1、PINP水平與二次妊娠組相比較,無(wú)顯著性差異(P0.05)。5.初次妊娠組、二次妊娠組組間比較胎兒股骨值,無(wú)顯著性差異(P0.05)。6.二次妊娠組的妊娠前、產(chǎn)后3個(gè)月、產(chǎn)后1年的骨密度值均較初次妊娠組略有減低,兩組比較存在顯著性差異(P0.05)。結(jié)論1.妊娠次數(shù)對(duì)孕婦的維生素D、Ca、P、Mg、CTX-1、PINP的水平及胎兒長(zhǎng)骨發(fā)育無(wú)明顯影響。2.妊娠次數(shù)對(duì)孕婦妊娠自身骨密度值有一定的影響,且產(chǎn)后3個(gè)月,孕婦的骨密度值處于最低水平。
[Abstract]:Objective to study the bone status of pregnant women and the development of fetal long bones in the same period by measuring the biomarkers of bone metabolism and bone mineral density in women with first and second pregnancies. To analyze whether the number of pregnancies will have an effect on fetal bone and pregnant women's own bone mineral density. Methods from February 2016 to December 2016, 520 pregnant women were randomly selected from the General Hospital of Ningxia Medical University. According to the exclusion criteria, 16 cases were excluded and the remaining 504 cases were selected as the study objects. According to the number of pregnancies, pregnant women were divided into two groups, first pregnancy 455 cases, second pregnancy 49 cases. The bone metabolic markers and fetal long bone were compared between the two groups. There were 104 cases of gastrocnemius spasm, 91 cases of first pregnancy and 13 cases of secondary pregnancy.
