【摘要】：目的探討卵巢原發(fā)性黑色素瘤的臨床病理學特征及鑒別診斷。方法對l例卵巢畸胎瘤進行大體檢查、光鏡觀察及免疫組化EnVision法檢測,并復習相關文獻。結(jié)果卵巢腫物表面光滑,切開囊實性,部分區(qū)域有脂質(zhì)及毛發(fā),部分區(qū)域灰黑色、質(zhì)軟、朽脆。鏡下腫瘤細胞中等大小,呈圓形及多邊形,胞質(zhì)紅染、透亮,核圓形、卵圓形、豆瓣樣,核仁明顯,染色質(zhì)粗糙,雙核細胞、多核細胞及瘤巨細胞易見。腫瘤細胞排列成片狀、巢狀、腺泡狀,光鏡下瘤組織內(nèi)見多處片狀壞死,另見細胞內(nèi)外多量黑色素顆粒。免疫表型:S-100、HMB-45、vimentin均陽性,CK、PLAP、Inhibin、CD45、desmin均陰性,Ki-67增殖指數(shù)約60%。結(jié)論卵巢原發(fā)性黑色素瘤是一種相對罕見的惡性腫瘤,其診斷需結(jié)合組織學形態(tài)及免疫表型。
[Abstract]:Objective to investigate the clinicopathological features and differential diagnosis of primary melanoma of ovary. Methods one case of ovarian teratoma was examined by light microscopy and envision immunohistochemical method. Results the surface of the ovarian tumor was smooth, the cyst was cut, the lipid and hair were found in some areas, and the areas were gray and black, soft and brittle. Under microscope, the tumor cells were of medium size, round and polygonal. The cytoplasm was red stained, transparent, round, oval, pea like, nucleolus obvious, rough chromatin, binuclear cells, multinucleated cells and tumor giant cells. The tumor cells were arranged in flake, nest and acinar shape. There were many pieces of necrosis and melanin granules inside and outside the tumor tissue under light microscope. Immunophenotypes of HMB-45 vimentin were all positive for CKP PLAPAPN, CD45 and desmin were negative for Ki-67 proliferative index (Ki 67). Conclusion Primary melanoma of ovary is a relatively rare malignant tumor and its diagnosis should be combined with histological morphology and immunophenotype.
