【摘要】：目的探討不影響宮腔形態(tài)的肌壁間子宮肌瘤對體外受精-胚胎移植(IVF-ET)妊娠結(jié)局的影響及隨訪觀察。方法選取于生殖中心行IVF-ET,不孕合并超聲檢查示不影響宮腔形態(tài)的肌壁間子宮肌瘤患者91例,共行胚胎移植100個周期(觀察組),抽取同期行IVF-ET治療的單純輸卵管性不孕不合并子宮肌瘤患者100例(對照組)。對兩組的一般情況、控制性超排卵情況、體外受精參數(shù)和妊娠結(jié)局進行回顧性對照分析。結(jié)果兩組的基本情況差異無統(tǒng)計學(xué)意義。觀察組與對照組的胚胎種植率(26.7%vs 30.5%)、臨床妊娠率(48.0%vs51.0%)、自然流產(chǎn)率(20.8%vs 31.4%)、活產(chǎn)率(32.0%vs 28.0%)、早產(chǎn)率(9.4%vs 7.1%)差異無統(tǒng)計學(xué)意義(P0.05)。隨訪示觀察組和對照組的剖宮產(chǎn)率為81.3%(26/32)和71.4%(20/28),差異無統(tǒng)計學(xué)意義(P0.05)。觀察組剖宮產(chǎn)術(shù)中行肌瘤剔除者超過半數(shù)(11/20),剔除的肌瘤大小為2~5 cm,無出現(xiàn)術(shù)中大出血。結(jié)論不影響宮腔形態(tài)的肌壁間子宮肌瘤對IVF-ET臨床妊娠結(jié)局無明顯不良影響。
[Abstract]:Objective to investigate the effect of intramural myoma without influence on the pregnancy outcome of IVF-ETS in vitro fertilization-embryo transfer (IVF-et). Methods the IVF-ETS were performed in the reproductive center and 91 patients with intramural hysteromyoma who had no effect on the shape of uterine cavity were examined by ultrasonography. A total of 100 cycles of embryo transfer were performed (observation group, 100 cases of simple tubal infertility with uterine leiomyoma treated with IVF-ET at the same time) (control group). The general condition, controlled hyperovulation, in vitro fertilization parameters and pregnancy outcome were analyzed retrospectively. Results there was no significant difference in basic conditions between the two groups. There was no significant difference between the observation group and the control group in embryo implantation rate (26.7 vs 30.5), clinical pregnancy rate (48.0 vs 51.0), spontaneous abortion rate (20.8 vs 31.41%), live birth rate (32.0% vs 28.0%), premature delivery rate (9.4% vs 7.1%). The rate of cesarean section in the observation group and the control group was 81.3 / 32 and 71.4 / 20 / 28 respectively. There was no significant difference between the two groups (P 0.05). In the observation group, more than half of the patients underwent leiomyoma removal during cesarean section, and the size of the leiomyoma removed was 2 ~ 5 cm. There was no intraoperative hemorrhage. Conclusion there is no significant adverse effect of intramural myoma on the pregnancy outcome of IVF-ET.
