妊娠糖尿病患者臍血炎癥因子水平及其相關(guān)性研究

【摘要】:背景:妊娠糖尿。╣estational diabetes mellitus, GDM)患者子代代謝綜合征(metabolic syndrome, MetS)發(fā)病風(fēng)險增加。高敏C反應(yīng)蛋白(high sensitivity C-reactive protein, hs-CRP)及白細胞介素6(interleukin-6, IL-6)是常見的炎癥因子,與代謝性疾病的發(fā)生密切相關(guān)。但目前關(guān)于GDM患者臍血hs-CRP、IL-6水平及其與子代營養(yǎng)狀態(tài)及對胰島素的敏感性的相關(guān)性研究仍很少。 目的:探討GDM患者臍血hs-CRP、 IL-6水平及其與子代營養(yǎng)狀態(tài)及對胰島素的敏感性的相關(guān)性。 方法:2009年-2011年于我院行剖宮產(chǎn)孕婦,,其中26名為GDM患者,28名為糖耐量正常(normal glucose tolerance, NGT)者。測定臍血hs-CRP、IL-6、血糖及胰島素水平,使用Ponderal指數(shù)(ponderal index,PI)評估子代營養(yǎng)狀態(tài),胰島素穩(wěn)態(tài)模型(homeostasis model assessmentof insulin resistance, HOMA-IR)評估對胰島素的敏感性。 結(jié)果:GDM組HOMA-IR、胰島素水平、PI均較對照組顯著增加。兩組臍血hs-CRP水平及IL-6水平無顯著統(tǒng)計學(xué)差異。HOMA-IR及臍血胰島素均與子代出生體重及PI正相關(guān)。NGT組臍血IL-6水平與子代出生體重負相關(guān),與hs-CRP正相關(guān),但校正孕婦年齡、孕前體重指數(shù)(body mass index, BMI)、胎齡及子代性別后上述相關(guān)性不再成立。GDM組臍血hs-CRP、IL-6水平與子代營養(yǎng)狀態(tài)及胰島素敏感性無相關(guān)性。 結(jié)論:1. GDM患者子代出生時即存在較NGT者子代高的胰島素水平,且對胰島素的敏感性下降。 2.臍血胰島素水平及對胰島素的敏感性與子代營養(yǎng)狀態(tài)密切相關(guān)。 3. GDM患者及NGT者臍血hs-CRP及IL-6水平與子代營養(yǎng)狀態(tài)及對胰島素的敏感性均無顯著相關(guān)性。
[Abstract]:Background: metabolic syndromes (MetSs) in offspring of gestational diabetes mellitus, GDM) patients with gestational diabetes mellitus are at increased risk. Gao Min C-reactive protein high sensitivity C-reactive protein (hs-CRP) and interleukin-6 (IL-6) are common inflammatory factors, which are closely related to the occurrence of metabolic diseases. However, there are few studies on the relationship between the level of hs-CRPU IL-6 in umbilical cord blood and the nutritional status of offspring and insulin sensitivity in patients with GDM. Objective: to investigate the relationship between the levels of hs-CRP, IL-6 in umbilical cord blood and the nutritional status of offspring and insulin sensitivity in patients with GDM. Methods: cesarean section was performed in our hospital from 2009 to 2011, of which 26 were GDM patients and 28 were normal glucose tolerance, NGT) patients with normal glucose tolerance. The levels of hs-CRP IL-6, blood glucose and insulin in umbilical cord blood were measured. The nutritional status of offspring was evaluated by Ponderal index. The homeostasis model assessmentof insulin resistance, HOMA-IRR was used to evaluate the sensitivity of insulin to insulin. Results HOMA-IRand insulin level and Pi were significantly increased in the weight GDM group compared with the control group. There was no significant difference in hs-CRP and IL-6 levels between the two groups. HOMA-IR and insulin were positively correlated with birth weight and Pi. The IL-6 level of cord blood in NGT group was negatively correlated with birth weight of offspring, and positively correlated with hs-CRP, but the age of pregnant women was adjusted. Body mass index (BMI), gestational age and sex of offspring were no longer correlated with the level of hs-CRPU IL-6 in cord blood of GDM group. There was no correlation between hs-CRPU IL-6 level and nutritional status and insulin sensitivity of offspring in GDM group. Conclusion 1. There were higher insulin levels and lower insulin sensitivity in GDM offspring than in NGT offspring. 2. Cord blood insulin level and insulin sensitivity are closely related to the nutritional status of the offspring. 3. There was no significant correlation between the levels of hs-CRP and IL-6 in cord blood of GDM patients and NGT patients and the nutritional status and insulin sensitivity of their offspring.
