【摘要】：目的探討反復(fù)種植失敗(recurrent implantation failure,RIF)患者子宮動脈血流及其相關(guān)因素的特點(diǎn)。方法選取2012年3月至2013年8月于煙臺毓璜頂醫(yī)院生殖醫(yī)學(xué)科因輸卵管因素行體外受精-胚胎移植(in vitro fertilizationembryo transfer,IVF-ET)治療的RIF患者(RIF組,n=40)為研究對象;同期IVF-ET治療首次助孕成功的患者(首次IVF助孕成功組,n=40),首次IVF-ET助孕治療失敗患者(首次IVF助孕失敗組,n=40)為對照組,進(jìn)行回顧性分析。于注射人絨毛膜促性腺激素(human chorionic gonadotropin,HCG)日行經(jīng)陰道彩色多普勒超聲檢查,測量子宮動脈搏動指數(shù)(uterine artery pulsatility index,UAPI)、子宮內(nèi)膜厚度,并同時測定血雌二醇及孕酮值。比較三組間相關(guān)指標(biāo)的差異。結(jié)果 RIF組患者UAPI 3.3±0.4,顯著高于同期IVF-ET治療首次助孕成功組2.3±0.5及失敗組UAPI 2.4±0.4,差異有統(tǒng)計學(xué)意義(P0.05),而IVF-ET治療首次助孕成功組與失敗組相比,差異無統(tǒng)計學(xué)意義(P0.05)。3組間的子宮內(nèi)膜厚度、雌二醇及孕酮值差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論 HCG日測定UAPI有助于評價子宮內(nèi)膜容受性,預(yù)測種植率。
[Abstract]:Objective to investigate the characteristics of uterine artery blood flow and its related factors in patients with recurrent implantation failure. Methods from March 2012 to August 2013, patients with RIF treated with in vitro fertilization and embryo transfer in vitro fertilizationembryo transfer IVF-ETwere selected from the Department of Reproductive Medicine, Yuzhouding Hospital, Yantai, as the study objects. At the same time, the patients who were successfully treated with IVF-ET for the first time (the first successful IVF group) and the first failed IVF-ET group (the first failed IVF group) as the control group were retrospectively analyzed. Human chorionic gonadotropin (HCG) was injected with human chorionic gonadotropin (HCG). The uterine artery pulsatile index, endometrial thickness and serum estradiol and progesterone were measured by transvaginal color Doppler ultrasonography. To compare the differences of relative indexes among the three groups. Results the UAPI of RIF group was 3.3 鹵0.4, which was significantly higher than that of IVF-ET group (2.3 鹵0.5) and failed group (UAPI 2.4 鹵0.4). The difference was statistically significant (P 0.05), while that of IVF-ET group was higher than that of failed group. There was no significant difference in endometrial thickness, estradiol and progesterone between the two groups (P 0.05). Conclusion HCG day measurement of UAPI is helpful to evaluate endometrial receptivity and predict implantation rate.
