【摘要】：目的通過對輸卵管妊娠患者施行不同的手術(shù)治療,觀察不同的手術(shù)方法對其卵巢儲備功能的影響。方法選擇2015年1月至2017年3月于蚌埠醫(yī)學(xué)院第一附屬醫(yī)院就診,因輸卵管妊娠需手術(shù)治療的患者120例,將其分為A、B、C三組,A組行腹腔鏡下輸卵管切除術(shù),B組行腹腔鏡下輸卵管切開取胚術(shù),C組行腹腔鏡下輸卵管部分切除術(shù)。同時選取40名健康體檢者作為對照組,分別檢測3組患者術(shù)后1、3個月經(jīng)周期及健康體檢者的血清卵泡刺激素(Follicle stimulating hormone,FSH)、黃體生成素(Luteinizing hormone,LH)、雌二醇(Estradiol,E2)、孕酮(progesterone,P)、睪酮(testosterone,T)水平。并檢測3組患者術(shù)前,術(shù)后1、3個月經(jīng)周期,健康體檢者的抗苗勒氏管激素(Anti Mullerian hormone,AMH)水平。采用經(jīng)陰道多普勒超聲技術(shù),分別檢測患者兩側(cè)卵巢的竇卵泡數(shù)(antral follicle count,AFC)。結(jié)果四組患者的年齡,停經(jīng)時間,血清絨毛膜促性腺激素等一般資料無統(tǒng)計學(xué)差異(P0.05)。A組患者術(shù)后1、3個月經(jīng)周期的FSH、LH水平比對照組升高,差異有統(tǒng)計學(xué)意義(P0.05)。E2、P、T、AMH水平較對照組降低(P0.05);B、C組與對照組相比,其FSH、LH、E2、P、T、AMH水平均無明顯差異(P0.05),且A組術(shù)后1個月經(jīng)周期與術(shù)后3個月經(jīng)周期的數(shù)據(jù)相比,無明顯差異(P0.05)。A組健側(cè)卵巢與手術(shù)切除輸卵管的患側(cè)卵巢相比,AFC明顯減少(P0.05)。結(jié)論腹腔鏡下輸卵管切除術(shù)對卵巢儲備功能有影響,輸卵管切除術(shù)后,導(dǎo)致卵巢儲備功能下降,腹腔鏡下輸卵管切開取胚術(shù)及腹腔鏡下輸卵管部分切除術(shù)對卵巢儲備功能無明顯影響。
[Abstract]:Objective to observe the effect of different surgical methods on ovarian reserve function in patients with tubal pregnancy. Methods from January 2015 to March 2017, 120 patients who needed surgical treatment for tubal pregnancy in the first affiliated Hospital of Bengbu Medical College were selected. Group A was divided into three groups: group A underwent laparoscopic salpingotomy and group B underwent laparoscopic oviduct incision and embryo extraction. Group C underwent laparoscopic partial salpingotomy. Serum follicle stimulating hormone (Follicle stimulating hormoneation), luteinizing hormone (LHH), estradiol (Estradiolium), progesterone (progesterone), testosterone (T) were measured in 40 healthy persons as control group after 1, 3 menstrual cycles and 1, 3 menstrual cycles after operation, and the levels of serum follicle stimulating hormone (Follicle stimulating), luteinizing hormone (LHH), estradiol (Estradiol), progesterone (progesterone), testosterone (T) were measured. The levels of anti-Mullerian Mullerian hormone (Anti Mullerian hormoney) were measured before operation, 1 and 3 menstrual cycles after operation in the three groups. Transvaginal Doppler ultrasound was used to detect antral follicle count of antral follicle. Results there was no significant difference in age, menopause time and serum chorionic gonadotropin between the four groups. Compared with the control group, there was no significant difference in the levels of TAMH between the two groups, and the data of one menstrual cycle and three menstrual cycles in group A were significantly lower than those in the control group (P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05, P 0.05 respectively. There was no significant difference in AFC between the normal and the affected oviducts in group P0.05A, and the number of AFC in group A was significantly lower than that in group A (P 0.05). Conclusion Laparoscopic salpingotomy has an effect on ovarian reserve function, and the ovarian reserve function decreases after salpingotomy. Laparoscopic oviduct incision and partial salpingotomy had no significant effect on ovarian reserve function.
