【摘要】：目的總結(jié)分析不同手術(shù)方式對(duì)Ⅰ期子宮內(nèi)膜癌患者的預(yù)后,以選擇最佳的外科治療方式。方法回顧性分析上海交通大學(xué)醫(yī)學(xué)院附屬瑞金醫(yī)院1995年1月至2008年3月收治的子宮內(nèi)膜癌患者98例,術(shù)前診斷為Ⅰ期。根據(jù)手術(shù)方式分為兩組,A組68例行全子宮雙附件切除,B組30例行全子宮雙附件切除+盆腔腹主動(dòng)脈旁淋巴結(jié)清掃術(shù),術(shù)后隨訪96~254個(gè)月。結(jié)果 98例隨訪率100%。93例最終分期為Ⅰ期的內(nèi)膜癌病例中,A組5年無(wú)瘤生存率88.2%,B組84.0%;術(shù)后復(fù)發(fā)10例(10.8%),其中A組7例,B組3例,兩組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。復(fù)發(fā)者為ⅠA(1/2肌層)G2(腫瘤2 cm)、ⅠB G2、ⅠB G3,及非子宮內(nèi)膜樣腺癌。淋巴結(jié)清掃組5例分期上升為ⅢC期,預(yù)后不佳。結(jié)論對(duì)于早期子宮內(nèi)膜腺癌(ⅠA期),腫瘤細(xì)胞分化良好(G1~G2),且無(wú)相關(guān)高危因素的患者,單純行全子宮雙附件切除術(shù)是最佳治療方案。
[Abstract]:Objective to summarize and analyze the prognosis of patients with stage I endometrial carcinoma by different surgical methods and to select the best surgical treatment. Methods 98 cases of endometrial carcinoma treated in Ruijin Hospital affiliated to Shanghai Jiaotong University Medical College from January 1995 to March 2008 were retrospectively analyzed. According to the operation mode, 68 patients in group A underwent total hysterectomy and 30 patients in group B underwent pelvic abdominal abdominal aortic lymph node dissection.
