【摘要】：【目的】探討系統(tǒng)性紅斑狼瘡(SLE)合并妊娠患者胎兒早產(chǎn)的臨床特點(diǎn)以及危險(xiǎn)因素�！痉椒ā炕仡櫺苑治鲋猩酱髮W(xué)附屬第一醫(yī)院收治的251例SLE患者共計(jì)263例次合并妊娠臨床資料進(jìn)行回顧性分析。多因素分析采用Logistic回歸模型進(jìn)行�！窘Y(jié)果】SLE孕婦妊娠時(shí)平均年齡為(28.6±3.9)歲(18~41歲)。共有188例(71.5%)活胎分娩,其中足月產(chǎn)132例(70.2%),早產(chǎn)56例(29.8%),另有75例(28.5%)發(fā)生妊娠丟失。34周的早產(chǎn)發(fā)生率為39.3%,≥34周的早產(chǎn)發(fā)生率為60.7%。早產(chǎn)的原因依次為治療性早產(chǎn)(38例),自發(fā)性早產(chǎn)(10例)和未足月胎膜早破早產(chǎn)(8例)。早產(chǎn)兒死亡率為3.6%,均發(fā)生于34孕周的胎兒。胎兒臍動(dòng)脈血流彩色多普勒超聲檢查提示早產(chǎn)兒臍動(dòng)脈血流S/D值高于足月產(chǎn)兒(P㩳0.05)。多因素分析顯示早產(chǎn)的危險(xiǎn)因素是高血壓(OR=5.7,95%CI 2.4~13.5,P0.001)和低蛋白血癥(OR=4.1,95%CI 1.7~9.8,P=0.002)�！窘Y(jié)論】SLE合并妊娠胎兒早產(chǎn)率高,治療性早產(chǎn)是主要原因,34孕周的早產(chǎn)兒死亡率高。SLE患者妊娠期間合并高血壓、低蛋白血癥可增加胎兒早產(chǎn)的危險(xiǎn)。胎兒臍動(dòng)脈血流彩色多普勒超聲可作為孕期監(jiān)測(cè)手段。
[Abstract]:[objective] to investigate the clinical characteristics and risk factors of fetal preterm delivery in patients with systemic lupus erythematosus (SLE) complicated with pregnancy. [methods] A retrospective analysis of #number0# cases of SLE patients in the first affiliated Hospital of Sun Yat-sen University was made. The clinical data of pregnancy were analyzed retrospectively. Multivariate analysis was carried out by Logistic regression model.
